PURPOSE:The aim of the study was to compare the clinical effects of a modified socket shield technique(SST)and the conventional immediate implant placement in esthetic zone through a prospective clinical case-control study,and also to explore whether the SST could acquire better final clinical outcomes.METHODS:Twenty-four patients with one hopeless upper incisor were consecutively enrolled at the Implant Department from September 2016 to September 2017 and scheduled for immediate implant placement.Twelve patients underwent a modified SST(experimental group)while the others underwent conventional procedures(control group).Radiographic and clinical data were obtained preoperatively,immediately after surgery,and 12 months after surgery.SPSS 21.0 software was used for statistical analysis.To compare the same group at different time points,the paired t-test was used if the data was normally distributed and the variance was homogenous,otherwise,the Wilcoxon signed rank test was applied.The comparison between the groups at the same time point was performed by independent-sample t test when normality and homogeneity of variance were satisfied,unless non-parametric test was used.The measurement data were statistically analyzed with ?2 test.P <0.05 was considered statistically significant.RESULTS:One year after operation,the success rate of implants was 100% in both groups.The PES scores of the experimental group and the control group before extraction(T0)were 13.17±0.83 and 13.08±0.79 respectively,without significant difference between them(P>0.05);while after one year(T2),the scores were 13.25±0.75 and 11.83±0.94 with significant difference(P<0.05).The absorption of the labial bone plates at 1mm,3mm,and 5mm to the implant shoulder was 0.04±0.13 mm,0.05±0.13 mm,and 0.11±0.26 mm respectively,and the control group was 0.85±0.45 mm,0.75±0.37 mm,and 0.65±0.35 mm,the differences were all statistically significant(P<0.05).The patients in the experimental group were more satisfied with the final esthetic outcomes than the control group(P <0.05).CONCLUSION : Compared with the traditional immediate implantation,the modified SST is advantageous over the conventional immediate implant placement in terms of buccal bone and soft tissue maintenance and does not affect the success rate of the implants.
