| Objective:To observe the safety and effectiveness of treating AECOPD(deficiency of lung and kidney,internal obstruction of phlegm and turbidities)with the method of removing phlegm by warming Yang.Methods:Seventy-two patients with AECOPD(deficiency of lung and kidney,internal obstruction of phlegm and turbidities)were selected from the pulmonary disease department of Yunnan Hospital of Traditional Chinese Medicine.They were randomly divided into the treatment group and the control group at a ratio of 1:1,with36 cases in each group.The control group was treated with conventional western medicine,and the treatment group was additively treated with Qifu Pingchuan Decoction.The treatment course of the two groups was observed for 10 days.The efficacy indexes and safety indexes of patients before and after treatment were observed and recorded,and the statistical software SPSS26.0 was used for analysis.Results:In this study,a total of 5 cases were excluded or shed,and 67 patients completed the study,including 34 patients in the treatment group and 33 patients in the control group.The specific research results are as follows:1.Total effective rateThe total effective rate was 85.29%in the treatment group(34 cases)and 75.76%in the control group(33 cases),The two effective rates were compared between groups(P=0.042<0.05),the effective rate of the treatment group is better than the control group.2.Comparison of TCM syndrome scores After treatment,the total score of syndrome,the scores of major symptoms(cough,phlegm,wheezing,shortness of breath,chills)and the scores of minor symptoms(chest tightness,lethargy,stomach puffiness and fatigue)in 2 groups were improved to varying degrees(P<0.05),the treatment group was superior to the control group(P<0.05).3.Comparison of inflammatory indexesAfter treatment,the inflammatory indexes(WBC,hs-CRP,SAA,PCT)in 2 groups decreased to different degrees(P<0.05),the treatment group was superior to the control group(P<0.05).4.Comparison of lung functionAfter treatment,pulmonary function indexes(FEV1%,FEV1/FVC%)in 2 groups were improved to varying degrees(P<0.05),the treatment group was superior to the control group(P<0.05).5.Artery blood gas analysis and comparisonAfter treatment,the blood gas analysis indexes(PaO2,PaCO2)of the two groups were improved to varying degrees compared with those before treatment,the treatment group was superior to the control group(P<0.05).6.Comparison of Oxygenation indexAfter treatment,Oxygenation index in both groups was improved compared with that before treatment(P<0.05),the treatment group was superior to the control group(P<0.05).7.Comparison of CAT and MMRC scoresAfter treatment,CAT score and MMRC score of 2 groups were improved to different degrees(P<0.05),the treatment group was superior to the control group(P<0.05).8.Comparison of average length of hospital stayThe average length of hospital stay after treatment was shorter in the treatment group than in the control group(P<0.05).9.Safety analysisDuring the clinical observation,no adverse reactions occurred in patients of the two groups,and the relevant safety indicators were within the normal range,indicating good drug safety.CONCLUSIONS:Compared with western medicine alone,Qifu pingchuan decoction combined with western medicine in the treatment of AECOPD(deficiency of lung and kidney,internal obstruction of phlegm)can effectively shorten the length of hospital stay,improve clinical symptoms,reduce inflammatory indexes,improve lung function,arterial blood gas,CAT and MMRC scores.It is suggested that the efficacy and safety of integrated Chinese and western medicine in the treatment of AECOPD is better than that of western medicine alone. |
