Objective:By exploring the correlation between TCM tongue diagnosis and currentperception threshold(CPT)in type 2 diabetes(T2DM)patients,the paper provides reference for tongue diagnosis in the prevention and treatment of diabetic peripheral neuropathy(DPN),a chronic complication of T2DM.Methods:Subjects were 569 patients with type 2 diabetes who met the inclusion criteria and were admitted to the Department of Endocrinology and Metabolic Diseases of the Affiliated Hospital of Changchun University of Chinese Medicine from January 2020 to January 2021.The CPT results were divided into severe paresthesia group(group A),moderate paresthesia group(group B),mild paresthesia group(group C),and no obvious paresthesia group(group D)according to the severity. The tongue features were obtained by using the convenient tongue diagnostic instrument(DS01-G)produced by Shanghai Daoseng Company. The tongue characteristics of patients were recorded by double and double entry method and the correlation analysis was conducted. All the data were statistically processed by SPSS22.0 statistical software, and the obtained data were statistically analyzed by frequency statistics,X~2test and two-factor analysis of variance.Results:1.The syndrome characteristics and ratio of patients in each group were different:blood stasis syndrome was more common in the severe paresthesia group(58.5%),while the other three groups were more common in the deficiency of Qi and Yin syndrome, accounting for33.8%,46.2%and 40.0%,respectively.2.Each tongue features of each are not identical:current sensory threshold measurement results 4 severely abnormal group most of the patients with tongue as features for"strong blue tongue teeth marks, moss peel color yellow",see more atⅢsublingual collaterals,Ⅱdegrees change; Most of the syndromes were blood stasis syndrome and deficiency of Yin and Yang syndrome.Current sensory threshold measurement results moderately abnormal group of patients most tongue is characterized by"red tongue is purple prick,yellow-and-white dry moss",see more atⅡsublingual collaterals,Ⅰdegrees change; Most of the syndromes were deficiency of both Qi and Yin and blood stasis.Current sensory threshold measurement results mildly abnormal group of patients with most of the tongue is characterized by"red tongue of crack, moss flake color white",sublingual collaterals stasis degree is given priority to withⅠdegree change; The syndromes statistics mainly included deficiency of Qi and Yin and excessive heat syndrome of Yin deficiency. The main tongue features of patients in the group without obvious abnormality were"thin red tongue, yellow and white oily tongue",and the degree of sublingual varicose was mainly changed by 0degree.The syndromes statistics were mostly deficiency of both Qi and Yin,and excessive heat deficiency of Yin,which were similar to those in the group with mild abnormal current sensation threshold measurement results.3.The statistical results of two-factor ANOVA showed that the abnormal degree of CPT results showed that the factors P<0.05 between the groups, indicating that there were differences in the tongue image of each group. The characteristics of the tongue image were different with different CPT results. The intergroup factors in the characteristic area of tongue pattern were P<0.05,indicating that the intergroup factors in each characteristic area of tongue pattern had influence on the ratio of tongue pattern.Conclusion:1.Patients with type 1 and type 2 diabetes mellitus have different CPT results and different tongue features, and the two are correlated. The preliminary inference of CPT results from tongue image can provide effective reference for the early prevention of diabetic peripheral neuropathy,a chronic complication of type 2 diabetes mellitus.2.According to the tongue characteristics of patients with CPT abnormalities,it can be summarized as follows:the pathogenesis of diabetic peripheral neuropathy is a mixture of deficiency and deficiency, manifested as deficiency of Yin and fluid, loss of qi and Yang, and blood stasis throughout the disease. With the aggravation of the abnormal degree of CPT results, the blood stasis of the sublingual collaterals became more and more obvious. For patients with severe and moderate CPT abnormalities in type 2 diabetes, more attention should be paid to the application of promoting blood circulation and removing blood stasis in the prevention and treatment of diabetic peripheral neuropathy,in addition to the treatment of nourishing Yin and reinforcing Yang and tonifying qi and nourishing Yin.
