| Objective: to study the effect of mirror therapy combined with rehabilitation training on hand dysfunction of stroke patients and the appropriate treatment time.The incidence rate of stroke has been increasing in China and the world over the past 20 years.Most patients have sequelae of stroke,such as limb dysfunction.Among them,hand dysfunction is 90%,and hand function is more difficult than functional rehabilitation of lower limbs.Most patients feel depressed about limb dysfunction after onset,which limits limb function related activities and seriously damages the quality of life and rehabilitation intention of patients.Therefore,in the process of rehabilitation treatment of stroke sequelae,hand dysfunction rehabilitation is one of the biggest difficulties.Hand function rehabilitation is not an independent single treatment,but a complex system engineering.With the influx of some new methods and new ideas,mirror therapy,as a low-cost and easy-to-use rehabilitation treatment method,has been gradually applied in clinical practice.It is a rehabilitation treatment method formed by specific rehabilitation training combined with plane mirror.Simple mirror therapy can not only improve the upper limb and hand function of stroke patients,but also improve the activities of daily living.Studies have shown that mirror therapy combined with other treatments can not only improve limb function and activities of daily living,but also improve pain,anxiety and depression.Although there are many researches on mirror therapy in clinic,the standards of specific treatment instruments,action classification and treatment time have not been unified,and the mechanism needs to be further studied.Methods: A total of 30 stroke patients with hemiplegic hand dysfunction admitted in the Department of Rehabilitation Medicine,the first affiliated Hospital of Dalian Medical University,Liaoning Province from April 2020 to December 2020 were selected strictly according to the inclusion criteria and exclusion criteria.The patients were divided into three groups: mirror group 1(n = 10),mirror group 2(n = 10)and control group(n =10).The control group was given routine rehabilitation training,mirror group 1 was given routine rehabilitation training and mirror therapy for 2 weeks,and mirror group 2was given routine rehabilitation training and mirror therapy for 4 weeks.Before and after treatment,Brunnstrom staging,modified Barthel index(MBI)and Jebsen hand function test(JHFT)were used to evaluate the therapeutic effect of patients in each group.The data were analyzed by SPSS 26.0.the data differences within and between groups were compared.P<0.05 was considered statistically significant.Results:1.Before treatment,there was no significant difference in Brunnstrom stage,MBI and JHFT scores among the three groups.2.Intra-group comparison: The Brunnstrom stage and MBI score of the patients in the mirror group 1 after treatment were higher than those before treatment(P<0.05).JHFT score was lower than that before treatment(P<0.05).Brunnstrom stage and MBI score of the patients in the mirror group 2 after treatment were higher than those before treatment,and the JHFT score was lower than that before treatment,The differences were statistically significant(P<0.05).3.Comparison between groups: Brunnstrom staging and MBI score in the mirror group1 were higher than those in the control group,while JHFT score in the mirror group 1was lower than that in the control group(P<0.05).The Brunnstrom staging and MBI score of the mirror group 2 were higher than those of the control group,while the JHFT score of the mirror group 2 was lower than that of the control group(P<0.05).The Brunnstrom staging of the mirror group 2 was higher than that of the mirror group1(P<0.05).MBI score of the mirror group 1 was lower than that of the mirror group 2,the JHFT score of the mirror group 1 was higher than that of the mirror group 2,the difference was statistically significant(P<0.05).Conclusion:1.both MT and routine rehabilitation training are effective for stroke patients with hand dysfunction.2.The therapeutic effect of MT for 4 weeks on stroke patients with hand dysfunction was better than that of MT for 2 weeks. |
