Objective:To study the clinical value of main blood flow parameters in pregnant women and fetuses with hypertensive disorders complicating pregnancy in different stages by Ultrasonography.Methods:1.This study selected 60 control groups that are pregnant women with intrauterine pregnancy with single live fetus with gestational week of 27-39 weeks and ages 24-38years from July 2019 to November 2020.Pregnant women in these control groups were excluded from fetal malformations and various pregnancy complications.This study also collected 66 pregnant women diagnosed with preeclampsia in our hospital as examples who were intrauterine pregnancy with single live fetus with gestational week of 27-39 weeks and ages 20-39 years from July 2019 to November 2020.According to the criteria of hypertensive disorders complicating pregnancy recommended by the 8th edition of“Obstetrics and Gynecology”,the patients were divided into mild preeclampsia group(groupⅠ,n=36)and severe preeclampsia group(groupⅡ,n=30)as cases.2.The blood flow parameters of pregnant women and fetuses in the control and case groups are as follows:Fetal pulmonary vein(PV)and ductus venous(DV)ventricular systolic peak velocity S,ventricular diastolic peak velocity D,atrial systolic maximum reflux velocity A,pulsatility index(PI);the ratio of peak systolic flow velocity to diastolic peak velocity(S/D),resistance index(RI),PI of fetal middle cerebral artery;RI,PI,S/D of umbilical artery and RI,PI,S/D of uterine artery in pregnant women,and calculated fetal cerebroplacental ratio(CPR)=PIMCA/PIUA.The gestational age,neonatal weight and Apgar score at birth were recorded.3.The data were processed by the SPSS 22.0 statistical software package,and the measurement data were(?x±s).The independent sample t-test and analysis of variance was used between each group,and the difference was statistically significant with P value is smaller than 0.05(P<0.05).With the Logistic binary regression analysis,the correlations between fetal DV-A,PV-A,CPR,uterine artery PI of pregnant women and perinatal outcome were obtained.Results:1.Compared with the control group,DV-S,DV-D,DV-A in mild preeclampsia group and severe preeclampsia group all decreased and the PI increased,with statistical significance(P<0.05).There was no significant difference between mild preeclampsia group and severe preeclampsia group(P>0.05).2.Compared with the control group,PV-S,PV-D and PV-A in the severe preeclampsia group all decreased and PI increased,with statistical significance(P<0.05).Compared with mild preeclampsia group,PV-S,PV-D and PV-A in severe preeclampsia group all decreased,with statistical significance(P<0.05).There was no significant difference in PV-PI between the severe preeclampsia group and the mild preeclampsia group(P>0.05).There was no significant difference in PV-S,PV-D,PV-A,PV-PI between mild preeclampsia group and the control group(P>0.05).3.Compared with the control group,S/D,RI and PI of uterine artery in mild preeclampsia group and severe preeclampsia group increased,with statistical significance(P<0.05).There was no significant difference between mild preeclampsia group and severe preeclampsia group(P>0.05).4.Compared with the control group,S/D,RI,and PI of the middle cerebral artery in the severe preeclampsia group all decreased,with statistical significance(P<0.05).Compared with the control group,PI of the middle cerebral artery in the mild preeclampsia group decreased,with statistical significance(P<0.05).There was no significant difference in S/D,RI of middle cerebral artery between mild preeclampsia group and the control group(P>0.05).There was no significant difference in S/D,RI and PI of middle cerebral artery between mild preeclampsia group and severe preeclampsia group(P>0.05).Compared with the control group,S/D,RI,and PI of umbilical artery in the severe preeclampsia group increased,with statistical significance(P<0.05).There was no significant difference in S/D,RI and PI of umbilical artery between mild preeclampsia group and control group(P>0.05).There was no significant difference in S/D,RI and PI of umbilical artery between mild preeclampsia group and severe preeclampsia group.(P>0.05).5.Compared with the control group,CPR in mild preeclampsia group and severe preeclampsia group decreased,with statistical significance(P<0.05).The CPR of the severe preeclampsia group was lower than that of the mild preeclampsia group,with statistical significance(P<0.05).6.The prognosis of perinatal infants was correlated with fetal DV-A,PV-A,CPR and uterine artery PI of pregnant women.Conclusion:The blood flow parameters of fetal ductus venous,pulmonary vein,umbilical artery,middle cerebral artery and uterine artery of pregnant women can reflect the fetal intrauterine conditions and predict adverse outcomes in different stages of hypertensive disorder complicating pregnancy.If combined with the above blood flow parameters for comprehensive analysis,clinicians can obtain more and more reliable references for timely and effective intervention,thereby improving the prognosis of the perinatal infant.
