Objective:The purpose of this study is to evaluate the feasibility of High-resolution Magnetic Resonance Angiography(HR-MRA) in the display of the Middle Cerebral Artery(MCA) and Penetrating Artery. Discussion High-resolution Magnetic Resonance Angiography(HR-MRA)in diagnosing the etiology in patients with infarction at the lenticulostriate(LSA)artery territory,and characterize the wall and plaque of Middle Cerebral Artery(MCA)atherosclerotic between cerebral infarction and TIA patients.Methods: T1 WI,T2WI,DWI and HR-MRA were applied to fourty healthy subjects.The number of penetrating artery,the wall and signal of the MCA on HR-MRA were observed by two physicians independently. The Wilcoxon test was used to compae the number of penetrating artery?T1WI,T2 WI,DWI and HR 3D-TOF-MRA were applied to twenty-eight patients has infarction at the territory which supplied by lenticulostriate(LSA)artery. The number and route of the penetrating artery,and its relationship between infarction zone were observed by two physicians independently. The Wilcoxon test was used to compare the number of penetrating artery?T1WI,T2 WI,DWI and HR-MRA(include 3D-TOF-MRA and CUBE T1) were applied to thirty-three patients with Middle Cerebral Artery(MCA)atherosclerosis. Divided into two groups,symptomatic MCA stenosis. And TIA.The location, rate of stenosis, degree of stenosis, shape of arteriosclerosis plaque, degree of enhancement were observed by two physicians independently. Measuring total vessel area(TVA) and lumen area(Lumen Area, LA)at the stenosis and total vessel area(TVA) at the adjacent reference position, calculate the rate of reconstruction rate.The x2 test was used to compare the degree of stenosis and enhancement. The x2 test was used to compare total vessel area(TVA) and lumen area(Lumen Area, LA)at the stenosis,total vessel area(TVA) at the adjacent reference position and reconstruction rate.Result:On 3D-TOF-MRA:The bilateral Heubner return artery(including the medial striate arteries, MSA) are 0-3, the median is 1; the Lenticulostriate(LSA) on the left artery are 0-7, the median is 3; the Lenticulostriate(LSA) artery on the right are 0-5, the median is 3. The bilateral Anterior Choroidal Artery(ACh A) are 1-2, the median is 1. The Thalamoperforate Artery(TPA)are 1-2, the median is 1.On CUBE T1: the Lenticulostriate(LSA) on the left artery are 0-5, the median is 3; the Lenticulostriate(LSA) artery on the right are 0-5, the median is 3.The MCA traveling naturally, the wall of and luminal of the vessel shows clearly.All the branches from Internal Carotid Artery(ICA), Thalamotuberal Artery(TTA), Thalamogeniculate Artery(TGA), Medial Posterior Choroidal Artery(MPCh A), Lateral Posterior Choroidal Artery(LPCh A) are not shown.The number of Lenticulostriate(LSA) on the left in patients has infarction at the territory which supplied by lenticulostriate(LSA)artery are 0-5, the median is 2,the opposite side is 1-6, the median is 3. The number of Lenticulostriate(LSA) on the right in patients has infarction at the territory which supplied by lenticulostriate(LSA)artery are 0-5, the median is 2, the opposite side is 1-5, the median is 3.The LA(2.14±1.55mm2vs2.72±1.26mm2)and total vessel area(TVA)(8.13±1.60mm2vs8.10±1.42mm2) at the adjacent reference position was similar between the two groups;Compared with TIA group,symptomatic MCA stenosis had a lager total vessel area(TVA)(8.65 ± 1.93mm2 7.10±1.38mm2),reconstruction rate(1.07±0.11vs0.88±0.11) at the stenosis,higher enhancement in symptomatic MCA stenosis.Conclusions:1.HR 3D-TOF-MRA and CUBE T1 is completely noninvasive angiography, It can be used to display the middle cerebral artery and its perforating artery;2.HR 3D-TOF-MRA can help diagnosing the etiology in patients with infarction at the lenticulostriate(LSA)artery territory;3. HR-MRA can characterize the wall and plaque of Middle Cerebral Artery(MCA)atherosclerotic.
