Investigation On Application And Related Influence Factors Of Transient Elastography (Fibro Touch) In Decompensated Hepatitis B Cirrhosis

Objective: To discuss the application value and influencing factors of transient elastography in the diagnosis of decompensated hepatitis B cirrhosis,and to know whether there are differences in liver lesions between Han and Uygur people with hepatitis B cirrhosis.Methods: Selected patients with hepatitis B liver cirrhosis from September 2018 to June 2020 in our hospital.228 patients were enrolled.83 patients in the compensation group of hepatitis B liver cirrhosis,145 patients in the decompensation group.Test the two groups of AST,ALT,ALB,TBA,TBIL,GGT,BMI and other indicators,perform LSM measurement,compare the differences between the indicators in each group,and test the correlation between LSM and each indicator.Results:(1)the LSM value of compensated group and decompensated group was significantly different(P<0.05).The LSM value of Han in compensated group and decompensated group was lower than that of Uygur(P<0.05).(2)There was a statistically significant difference in gender,AST,GGT,ALT,TBIL,TBA,ALB,and LSM between the compensated group and the decompensated group of post-hepatitis B liver cirrhosis(P<0.05).Comparison of various indicators between Han and Uyghurs in the hepatitis B liver cirrhosis compensatory group and decompensated group showed significant differences in gender and BMI(P<0.05),and the BMI of Han patients was significantly lower than that of Uyghurs.(3)LSM was positively correlated with AST,ALT,TBA,TBIL,GGT,BMI,and negatively correlated with ALB,the difference was statistically significant(P<0.05).Logistic regression analysis showed that AST,ALT,TBIL may be risk factors for decompensation of liver cirrhosis after hepatitis B,and ALB may be a protective factor.(4)The ROC curve analysis showed that the critical value of LSM for diagnosing decompensated hepatitis B cirrhosis was 19.7k Pa,the area under the curve was 0.893[95%CI(0.853-0.934)],the sensitivity was 0.834,and the specificity was 0.800;the Han diagnosed hepatitis B cirrhosis the critical value of decompensated LSM is 19.3k Pa,the area under the curve is 0.914[95%CI(0.871-0.956)],the sensitivity is 0.822,and the specificity is 0.889;the critical value of decompensated LSM is 20.05 k Pa after diagnosis of hepatitis B in Uyghurs,the area under the curve is0.898[95%CI(0.828-0.969)],the sensitivity is 0.816,and the specificity is 0.862.Conclusions: Transient elastography can effectively evaluate the liver stiffness value of hepatitis B cirrhosis,and then define the stage of cirrhosis,but attention should be paid to the influence of liver function on it.There may be differences in LSM between Han and Uyghur,but further research is needed.
