Backgroud and ObjectiveImmune nephropathy is the general term for a group of chronic glomerular diseases,mainly including Henoch-schonlein purpuric nephritis(HSPN),lupus nephritis(LN),immunoglobulin A(Ig A)nephropathy,and the incidence of this type of nephropathy is increasing year by year,and the harm is serious,it can evolve into uremia,and there is a risk of death.Clarifying the clinical characteristics of patients with immune nephropathy has important guiding significance for clinical diagnosis and treatment.Immune mechanisms are involved in the occurrence and development of chronic kidney disease.However,the significance of serum immunoglobulin detection in the clinical diagnosis of various immune kidney diseases is still unclear,and it is necessary to conduct systematic research and analysis.This study explored the significance of serum immunoglobulin concentration in the clinical diagnosis of immune nephropathy,and hoped to provide a reference for the diagnosis and treatment of immune nephropathy.Materials and methodsA total of 95 patients with immune nephropathy who were diagnosed and treated in the First Affiliated Hospital of Zhengzhou University from January to December2019 were selected as the research objects.According to the type of immune nephropathy,they were divided into HSPN,LN,and Ig A nephropathy groups,including 32 cases in HSPN group,40 cases in the LN group and 23 cases in the Ig A nephropathy group.All patients met the diagnostic criteria for each disease.Another50 cases of healthy physical examination in this hospital were selected as the control group.Collect all research subjects’ age,gender,and clinical symptoms related data,and detect Ig G,Ig A,Ig M,three types of serum immunity Globulin and complement C3,C4,and detect 24-hour urinary protein quantity(24h UPQ),serum albumin(ALB),total cholesterol(TC),and triglyceride Triglyceride(TG),Serum Creatinine(Scr).Count the age,gender,symptoms,Ig G,Ig A,Ig M,C3,C4,24 h UPQ,ALB,TC,TG,Scr of each group of subjects for data analysis,and analyze Ig G,Ig A,Ig M,C3,C4 with Pearson correlation relationship with 24 h UPQ,ALB,TC,TG,Scr.Results1.The average age of the HSPN group,LN group,Ig A nephropathy group and control group was statistically significant(P < 0.05).The age of the HSPN group was≤ 18 years old;The age of the LN group was mostly between 19 and 45 years old;The Ig A nephropathy group had a wide and concentrated age distribution under 45 years of age.The age distribution of Ig A nephropathy group was widely concentrated in the under 45 years old.2.The difference in gender distribution between the four groups was statistically significant(P < 0.05).The HSPN group was dominated by men and the LN group was dominated by women.The Ig A nephropathy group had slightly more women than men and the control group had more uniform distribution of men and women.3.The symptoms of the three types of immune nephropathy patients are diverse.The differences in the incidence of edema,rash,abdominal pain,and arthralgia in the HSPN group,LN group,and Ig A nephropathy group are statistically significant(P <0.05).No statistical significance(P > 0.05).4.The Ig G in the LN group was significantly higher than the control group and other immune nephropathy groups(P < 0.05),the Ig G in the Ig A nephropathy group was significantly higher than the control group,HSPN group(P < 0.05),and the Ig G in the HSPN group was significantly lower than the control group(P < 0.05));Ig A in the LN group and Ig A nephropathy group was significantly higher than that in the control group and HSPN group(P < 0.05).There was no significant difference in Ig A comparison between the LN group and the Ig A nephropathy group(P > 0.05),and the Ig A in the HSPN group was significantly lower than the control Group(P < 0.05);Ig M level in Ig A nephropathy group was significantly higher than that in control group and other immune nephropathy groups(P < 0.05),Ig M level in LN group was significantly lower than control group(P < 0.05),HSPN group Ig M and control group The difference was not statistically significant(P > 0.05);C3 and C4 in the LN group were significantly lower than the control group and other immune nephropathy groups(P < 0.05)C3 and C4 of HSPN group and Ig A nephropathy group were significantly lower than those of control group(P < 0.05).There was no statistically significant difference between C3 and C4 of HSPN group and Ig A nephropathy group(P > 0.05).5.The study found that the 24 h UPQ,TC,TG and Scr levels of the HSPN group,LN group,and Ig A nephropathy group were significantly higher than those of the control group(P < 0.05),and ALB was significantly lower than the control group(P< 0.05).6.Serum Ig G,Ig A,Ig M were negatively correlated with 24 h urine protein and TC(P < 0.05).However,they were positively correlated with ALB(P < 0.05).Serum C3,C4 were negatively correlated with TC and TG(P < 0.05).Conclusions1.The decrease of Ig G and Ig A was the main clinical feature of HSPN,the increase of Ig G,the decrease of Ig M,C3 and C4 was the main clinical feature of LN,the increase of Ig M and Ig A was the main clinical feature of Ig A nephropathy.2.The level of serum immunoglobulin was closely related to urinary protein,ALB and blood lipid.3.The detection of serum immunoglobulin concentration can provide reference basis and clinical guidance value for the clinical diagnosis and treatment of immune nephropathy.
