Objective: To investigate the clinicopathological features and prognosis of IgA nephropathy and adult Henoch-Sch?nlein purpura nephritis.Methods: 643 cases of IgAN and 84 cases of HSPN were collected from August 2008 to December 2017 in Xinjiang Military Region General Hospital.Their clinical manifestations,laboratory examinations,renal pathology,treatment and prognosis were retrospectively analyzed.Results: The age of onset of HSPN was younger,the male ratio was higher,and the incidence of nephrotic syndrome was lower(P<0.05).IgAN patients had higher neutrophil/lymphocyte ratio,higher incidence of albumin reduction and a higher incidence of blood cholesterol,while HSPN patients had more urinary red blood cells,higher glomerular filtration rate,higher serum IgA and higher ratio of IgA/C3(P<0.05).In renal pathology,the proportion of crescent bodies and fibrocalcific bodies in HSPN cells was higher than that in IgAN(P<0.05).There was no significant difference in prognosis between the two groups(P<0.05).Compared with HSPN patients,IgAN patients were older,had higher incidence of nephrotic syndrome,fewer urinary red blood cells,lower ratio of IgA/C3 and lower eGFR(P<0.05);in renal pathology,the proportion of crescents and the incidence of interstitial vascular lesions were higher in HSPN patients than in HSPN patients(P<0.05).HSPN patients with urinary protein< 1g/d had a higher proportion of males,younger age and higher eGFR(P<0.05);there was no significant difference between IgAN group and HSPN group in renal pathology(P>0.05).Conclusion: Adult HSPN is younger and more common in males.The clinical manifestations of HSPN are similar.Renal pathological changes of HSPN are more acute.Hormone therapy alone is the main treatment in both groups,but there is no significant difference in prognosis between the two groups.
