| Objective To evaluate the accuracy of 6D computed-derived implant guides in China for dental implant surgery in edentulous,so as to provide the methods and bases for reducing deviation.Methods1.3D rapid prototyping technique was used to fabricate the implant guides for auxiliary surgical implantation in edentulous patients,and the position deviation between the preoperative design and the actual implant was measured.Fourteen edentulous patients with 100 implantation sites were measured.Preoperative cone-beam CT data were imported into the 6D Dental Planning Software(?)to design and made the implant guides.Digital guides were used to assist the implant placement.The preoperative design and postoperative CBCT were imported into the software for 3D reconstruction and registration,and then exported to Geomagic Studio software for analysis to obtain the deviation between the preoperative design and the implantation,so as to analyze the accuracy of the guides.The influence of different support methods,upper and lower mandibular,different implant methods,anterior and posterior teeth on the accuracy of the guide plate was analyzed.The source of deviation in the computed-derived implant guides for dental implant surgery in edentulous was analyzed,and the method of deviation control was studied.2.By improving the guide plate tools,sixteen cases of edentulous implantation with improved tool box were collected,including 94 implantation sites were measured,so as to further improve the accuracy of the implant guide plate.Results 1.General situations of accuracy:The distance deviation of implant neck center point was(0.83±0.27mm)and the horizontal deviation was(0.39±0.17mm).The distance deviation of implant bottom center point was(1.11±0.35mm)and the vertical deviation was(0.45±0.19mm).The angle deviation was(3.16±1.73°).2.There was no significant difference between the teeth-mucosal support guide plate and the mucosal support guide plate in the neck center deviation,horizontal deviation,bottom center deviation and depth deviation(P>0.05),and the teeth-mucosal support guide plate was slightly less in the Angle deviation than the mucosal support guide plate(P<0.05).3.The accuracy of the mandibular was slightly higher than that maxillary based on average,but there was no statistically significant difference(P>0.05).4.The accuracy of vertical implantation was slightly higher than that of oblique implantation based on average,but there was no significant difference(P>0.05).5.The accuracy of anterior teeth implantation site was slightly higher than that of the posterior teeth based on average,but there was no significant difference(P>0.05).6.There were significant differences in accuracy betweem New and old tool box of tooth-mucosal support guide plate in the implant neck center point and implant bottom center point(P<0.05).Both than the New and old tool box of mucosal support guide plate in the implant neck center point,horizontal deviation and implant bottom center point(P<0.05).Conclusion 1.The 6D computed-derived implant guide can significantly improve the accuracy and efficiency of dental implant surgery in edentulous.To reduce the operation time,reduce postoperative complications,and obtain better clinical effects.2.The accuracy of the teeth-mucosal support guide plate is higher than that the mucosal support guide plate.4.The accuracy of the new tool box without pressure plate is improved than the old.5.6D computed-derived implant guide is safe and reliable,which can improve the accuracy.However,there are still deviations,so during clinical operation,the operator should strictly control the judgment,and control the deviation in a smaller range,so as to obtain better clinical effect. |
