ObjectiveKawasaki disease(KD)is an inflammatory disease,and serum ferritin(SF)and peripheral blood lymphocyte to monocyte ratio(LMR)are considered as inflammatory markers.The target of this research is to seek the value of SF and LMR in the diagnosis of KD and the relationship between SF,LMR and coronary artery injury.MethodsThis is a retrospective analysis.Sixty children with Kawasaki disease who were admitted to the internal medicine ward of Shenyang Children’s Hospital during September 2018 to November 2020 and were confirmed for the first time and got standard treatment and follow-up were selected as the research subjects,as to the course of recovery,60 cases were divided into two parts: coronary artery lesion group(CAL group,6 cases)and non coronary artery lesion group(NCAL group,54 cases)depending on the presence of coronary artery injury;Sixty children with respiratory system infection with fever were voted as the febrile control group.Meanwhile,sixty healthy children were elected as the healthy comparison group who received health checkup.The datum of three groups of kids were gathered,including age,sex,history,diagnosis,treatment,laboratory examination(including acute stage blood routine,CRP,SF)and echocardiogram results of Kawasaki disease.We employ SPSS25.0 data analysis software to analytic comparison the disparity in SF and LMR between the KD group and the matched group,and among the CAL group and the NCAL group.The pertinence analysis of SF,LMR and other inflammatory indicators was carried out.Finally,ROC curve was drawn to determine the diagnostic efficacy of SF,LMR.P<0.05(bilateral)was reckoned statistically significant.Results1.Comparison of case data among the three groups(1)Among the three groups,there was no significant discrepancy in age and gender(P<0.05).There was no significant variation in thermal process between the Kawasaki disease group and the respiratory infection group at admission(P<0.05)The three groups met the principle of comparability.(2)Comparison of examination results:(1)SF in Kawasaki disease group was markedly exceeded than that in fever and healthy groups,while LMR was inferior to another two groups(P<0.001).(2)The dependency analysis of SF,LMR and blood routine indexes displayed that SF was positively related to WBC 、 NEU and CRP(P<0.05),and the coefficients of association were 0.519,0.587 and 0.451.LMR was negatively correlated with SF,WBC,NEU and CRP(P<0.05),and the correlation index were-0.333,-0.306,-0.433 and-0.368.These indicators have no correlation with HB and PLT(P>0.05).The acreage under the ROC curve of SF and LMR in the diagnosis of KD were 0.792 and 0.790(P<0.05).When the cut-off values of SF and LMR for the diagnosis of Kawasaki disease were 142.45ng/ml and 4.65,the sensitivity were 76.7% and 81.7%,and the specificity were 73.3% and 65.0%,respectively.The acreage under ROC curve of SF and LMR in the joint diagnosis of KD was 0.854,and its sensitivity and specificity were 76.7% and 80.8%.2.Comparison of general data,SF and LMR between CAL group and NCAL group(1)There was no significant difference in age and gender between the two groups(P>0.05).(2)Comparison of SF and LMR between the two groups: Compared with the NCAL group,SF was more advanced and LMR was lesser in the CAL group,but there was no statistical significance after examination(P>0.05).Conclusions1.In the acute stage of KD,the value of SF augmented and the value of LMR decreased,which have certain reference value for the diagnosis of KD,and the combined diagnostic value is slightly better.2.SF and LMR had no predictive effect on KD complicated with coronary artery injury.
