Knowledge Atlas And Clinical Observation Of Tongchang Decoction In Treating Early Postoperative Inflammatory Small Bowel Obstruction

Purpose:Early postoperative inflammatory small bowel obstruction(EPISBO)refers to early postoperative inflammatory small bowel obstruction(EPISBO),which is caused by surgical trauma or inflammation and exudates in the abdominal cavity,forming a mechanical and dynamic adhesive intestinal obstruction.Most of them can be cured by non-surgical treatment.Tongchang Decoction is a long-term empirical prescription of General Surgery of Hubei Provincial Hospital of Traditional Chinese Medicine.It is composed of rhubarb,mirabilite,citrus aurantium,magnolia officinalis,scutellaria,etc.It has the effects of tongli attacking,promoting qi and relieving pain.1.By reviewing and sorting out the literature related to postoperative intestinal obstruction,excavating the research subject,research hotspot and evolution trend of postoperative intestinal obstruction.2.This study aims to observe the clinical efficacy of Tongchang Decoction in the treatment of early postoperative inflammatory bowel obstruction,and to conduct objective evaluations.Method:1.Based on Gephi and Cite Space software,data mining is carried out on the related research literature of postoperative intestinal obstruction in the past 25 years,and the literature data is extracted to construct and analyze the knowledge map.2.According to the inclusion and exclusion criteria of the disease,51 patients with early postoperative inflammatory bowel obstruction admitted to the Department of Surgery(Hepatobiliary and Gastrointestinal Surgery)of Hubei Provincial Hospital of Traditional Chinese Medicine from November 2018 to December 2020 were selected.According to the random grouping of patients,there were 24 in the control group and 27 in the treatment group.The control group received basic treatment of intestinal obstruction:fasting,gastrointestinal decompression,anti-infection,parenteral nutrition support and other conventional methods of western medicine to promote gastrointestinal recovery,the treatment group based on the basic treatment of the control group,will Tongchang Decoction stomach Tube injection and traditional Chinese medicine retention enema treatment promote the recovery of gastrointestinal function.Observe the time of first hearing bowel sounds,the time of first passing gas and defecation through the anus,and the time of relief of abdominal pain after treatment with different treatments.And by observing the vital signs and adverse reactions of the subjects before and after medication,and inquiring about the specific medication experience,to evaluate its safety.Result:1.In the past ten years,the number of literatures in the field of postoperative intestinal obstruction research has generally fluctuated and increased;the journals with the most relevant research published in the literature sources are "Chinese Medical Guide","Henan Surgery Journal",etc.;The research institutions are mainly concentrated in Nanjing General Hospital of Nanjing Military Region,Tangshan People’s Hospital,Shengjing Hospital Affiliated to China Medical University,West China Hospital of Sichuan University,etc.The cooperation network is closely connected;the keyword "early postoperative inflammatory bowel obstruction" since 2000 Starting to highlight,"Compound Dachengqi Decoction" began to emerge in 2010;cluster keywords such as "Tumor Necrosis Factor-α","Nursing Intervention",and "Inflammatory Bowel Obstruction" are frequently studied.2.In the treatment group,22 cases were clinically cured,3cases were effective,1 case was effective,0 cases were ineffective,and the total effective rate was 100%;in the control group,16 cases were clinically cured,2 cases were effective,and 2 cases were effective.4 cases were ineffective,with a total effective rate of 83.3%.The treatment group was significantly better than the control group.After testing,P<0.05,the difference was statistically significant.After treatment,patients in the treatment group heard the bowel sounds for the first time,the first time to pass the anus,the time to defecate,and the time to relieve abdominal pain were better than those of the control group.After statistical analysis,the difference was statistically significant(P<0.05).Safety indicators: There were no obvious adverse reactions in all patients in this study,and no abnormalities were found in the vital signs of the two groups of patients before and after treatment.Conclusion:1.Research on postoperative intestinal obstruction has attracted more and more attention from researchers.Among them,"early postoperative inflammatory intestinal obstruction" and "Fufang Dachengqi Decoction" are research hotspots in the research field.In future research,The cooperation and exchange of researchers in different regions can be further strengthened,and the integration of regional resources needs to be further improved to jointly promote the development and progress of disciplines and the cross integration with other disciplines.2.Tongchang Decoction has obvious curative effect in the treatment of early postoperative inflammatory bowel obstruction,which is conducive to the elimination of intra-abdominal inflammation and the absorption of exudate,and can promote the gradual reduction of intestinal wall edema,promote the recovery of intestinal function,and significantly shorten the length of hospital stay,And can greatly improve the effectiveness of western medical treatment,reduce hospitalization costs,and alleviate the suffering of patients.It is an ideal treatment method.Tongchang Decoction is worthy of clinical application in the treatment of early postoperative inflammatory bowel obstruction.
