The Clinical Study Of External Fixator Combined With Chimu Lotion In Fracture Of Tibia And Fibula After Operation

Background:With the acceleration of industrialization and the rapidity and convenience of transportation,tibiofibular fracture plays an important role in the fracture of the whole body caused by industrial accidents and traffic accidents.With the advantages of small wound,simple operation and convenient disassembly,the external fixation frame has significant advantages in the fixation of tibia and fibula fracture.Chimu lotion is the traditional Chinese medicine characteristic of orthopaedics department in our hospital,which fully shows the advantages of simple,convenient,cheap and effective traditional Chinese medicine.Objective:To observe the clinical effect of external fixation combined with Chimu lotion in the treatment of tibia and fibula fracture,and to provide some clinical data for the treatment of fracture with traditional Chinese and western medicine.Methods:In this study,the patients who met the inclusion criteria and exclusion criteria were divided into two groups by random digital table method.The intervention measures of external fixation combined with warm water and external fixation combined with Chimu lotion were given respectively.three months after operation,the score of Johner-Wruh tibia fracture,the X-ray score of eschar growth,the V AS pain score and the difference rate of leg circumference were observed between the two groups before and after treatment.SF-36 quality of life score and the incidence of adverse events during follow-up.Results:①There was significant difference in the curative effect standard of Johner-Wruh tibia fracture between the routine group and the observation group treated with external fixation combined with Chiba lotion fumigation and washing,that is,32 cases were excellent,14 cases were good,12 cases were poor,2 cases were poor,the excellent and good rate was 90.32%,while 38 cases were excellent,18 cases were good,5 cases were good and 1 case was poor.The excellent and good rate was 76.67%,which showed that the excellent and good rate in the observation group was significantly higher than that in the routine group.②Three months after operation,X-ray reexamination showed that there was more eschar growth in both groups,and there was no significant difference in X-ray score between the two groups(P>0.05).③With the progress of treatment,the VAS pain score of the two groups decreased gradually,and the score 3 months after operation was significantly lower than that before operation,the difference was statistically significant(P<0.05),but there was no difference between the two groups before operation and 3 months after operation(P>0.05).④At 3 months after operation,the difference rate of leg circumference between the two groups was significantly higher than that before operation and 1 month after operation(P<0.05),and there was no significant difference between the two groups before operation,1 month after operation and 3 months after operation(P<0.05),but there was significant difference between the two groups at 2 months after operation(P<0.05),but there was no significant difference between the two groups before operation,1 month after operation and 3 months after operation(P<0.05),but there was no significant difference between the two groups before operation,1 month after operation and 3 months after operation(P<0.05).⑤Compared with the preoperative group,there was a significant improvement in SF36 scores in the two groups at 3 months(P<0.05).⑥There were no adverse events such as loosening of fixed needle,broken nail of fixed needle,perifascia syndrome,deep infection and fracture nonunion in both groups.The occurrence of delayed union of fracture was similar in the two groups.1 patient in the observation group had allergic dermatitis,the time of fumigation and washing was shortened,and the frequency of fumigation and washing was reduced.In the routine group,1 patient had superficial infection and was treated with routine debridement and dressing change.after one week of oral treatment with cephalosporins,the infection was controlled.Conclusion:The treatment of external fixation combined with redwood wash can effectively relieve the pain after tibial fibula fracture,promote the regression of limb swelling,improve the functional activity after tibial fibula fracture,and improve the quality of life of SF36 after fracture,which is a simple,effective and safe treatment for treatment of tibial fibula fracture,which is worth popularized in clinical practice.
