| Objective:Interstitial Lung Disease(ILD)is a group of heterogeneous diseases with interstitial hyperplasia and inflammatory cell infiltration as the main pathological changes.Pulmonary fibrosis(PF)is the most common type of interstitial lung disease.In traditional Chinese medicine,it mostly belongs to the category of"Fei Wei",The course of disease is generally progressive,and there is no effective clinical treatment drug.VEGF/PI3K/AKT signaling pathway is one of the important pathways related to angiogenesis.Astragaloside is a kind of monomer compound with the highest biological activity in Astragalus,which is one of the commonly used drugs in the clinical treatment of pulmonary fibrosis.In this experiment,astragaloside was given to intervene the pulmonary microvascular endothelial cells(PMVECs)after fibrosis.The protective effect of Astragaloside on the angiogenesis of pulmonary microvascular endothelial cells(PMVECs)was observed.In order to provide reference for clinical treatment,the relevant prescriptions and traditional Chinese medicines used in the treatment of interstitial lung disease were sorted out and classified.Methods:1.Clinical research:Sort out the outpatient medical records of pulmonary disease department of Shandong hospital of traditional Chinese medicine from January 2016 to June 2017,select 316 cases that meet the inclusion criteria of this study,316 prescriptions,use Excel software,establish data mining model and the frequency of drug use in the prescriptions,and conduct relevant analysis on drug use law.At the same time,select clinical typical medical records for case analysis.2.Experimental study:2.1 Establishment of lung microvascular endothelial cells(PMVECs)induced injury model and selection of astragaloside concentration.CCK-8 method was used to select different concentrations of inducer IL6 and astragaloside to act on PMVECs at different time,and the optimal concentration was selected;2.2 Based on VEGF/PI3K/Akt signal pathway,angiogenesis and the protective effect of Astragaloside on PMVECs were observed.They were divided into four groups:blank control group(group A),model group(group B),astragaloside+PMVECs+IL6 group(Group C),VEGF inhibitor group+PMVECs+IL6 group(Group D).After that,the proliferation of the cells was observed,the protein levels of VEGF,PI3K,Akt and mTOR were detected and compared by Western blot,and the mRNA levels of VEGF were measured and compared by qPCR.Results:1.Clinical research:Through the analysis of prescription drugs in 316 clinical cases,the most commonly used Chinese medicines are Qi-invigorating drugs,blood-activating and stasis-resolving drugs,and heat-clearing and phlegm-reducing drugs.Astragalus is the most frequently used,followed by ephedra,Qianhu,Baiqian and loquat leaves.In the analysis of typical medical records,the patients with interstitial lung disease treated by the method of strengthening vital energy and benefiting vital energy reported that the symptoms of respiratory tract were relieved and the times of aggravation of infection were also reduced.2.Experimental study:According to the results of CCK-8,the concentration of inducer IL-6(1 μg/ml)was set as the intervention concentration,and the concentration of astragaloside a(10 μm)was set as the experimental concentration.Compared with group A,IL-6 could inhibit the proliferation of PMVECs,decrease the expression of VEGF and PI3K.Compared with group A,group C can significantly promote the proliferation and migration of cells,increase the level of VEGF protein,and improve the expression of PI3K and other related factors.Conclusion:(1)Astragalus has a high frequency of application in the treatment of interstitial lung disease with traditional Chinese medicine.When my tutor treats interstitial lung disease,he should treat it according to syndrome differentiation,pay attention to invigorating qi and blood,and also use Astragalus in combination.At the same time,he should also give consideration to relieving cough and resolving phlegm,promoting blood circulation and removing blood stasis.It has a certain clinical effect,which is of great significance for guiding clinical medication.(2)Astragaloside can promote the proliferation and migration of PMVECs and the expression of VEGF and PI3K,which may play a role through VEGF/PI3K/Akt pathway. |
