| Objective : This study is to collect medical records of patients with cognitive impairment in different degrees to explore the research on the factors of TCM symptoms of cognitive disorders, and provide a certain reference for the understanding and diagnosis and treatment of cognitive disorders from the perspective of TCM syndromes. Methods : From December 2016 to May 2018,120 patients who were mainly complained of memory loss from the outpatient department and inpatient department of the Department of Neurology,Weifang City Hospital of Traditional Chinese Medicine were aged 55-85 years old, according to their etiology and severity Alzheimer’s disease(AD),vascular dementia(Va D),mild cognitive impairment(MCI),and vascular cognitive impairment(VCI)were divided into four groups,30 cases in each group.Record the clinical signs, general conditions, past medical history, risk factors, craniocerebral MRI or CT results of Chinese medicine, integrate according to the dementia syndrome scale, and calculate the distribution of different subtypes;after summarizing all clinical data, establish a database,Use SPSS19.0 statistical software package to process the data. Results :(1)The general data distribution of the four groups of patients: the average age of patients in AD group is(72.82 ± 9.59)years,14 male patients and 16 female patients;the average age of patients in MCI group is(68.87 ± 9.82)years,12 male patients,18 female patients;the average age of Va D patients was(71.31 ± 9,76)years old,19 male patients and 11 female patients;the average age of VCI group patients was(67.74 ± 9.16)years old,16 male patients, female 14 patients. The x2 test suggests that the degree of cognitive impairment of patients has a positive correlation with age, that is,as the age increases, the degree of cognitive impairment increases.(2)The scores of TCM symptom elements in the four groups of patients: from MCI,VCI to AD,Va D,kidney deficiency syndrome, spleen deficiency syndrome, blood deficiency syndrome, phlegm turbidity syndrome score gradually increased, that is,the change from mild cognitive impairment to dementia The symptoms of kidney deficiency syndrome, spleen deficiency syndrome and blood deficiency syndrome gradually increased;the scores of Qi deficiency syndrome,Yin deficiency syndrome and Yang deficiency syndrome gradually increased from MCI to AD stage.Symptoms of deficiency syndrome and yang deficiency syndrome are gradually increasing.(3)The proportion of TCM symptom elements of the four groups of patients is the most.According to the proportion of TCM symptom factors, the cognitive impairment disorders account for a relatively large number. The symptoms are kidney deficiency syndrome, marrow reduction syndrome, phlegm turbidity syndrome,qi deficiency syndrome and blood deficiency syndrome.(4)The overall distribution of TCM syndrome elements in the four groups showed the same trend(P> 0.05),there was no significant statistical difference, but the four groups of patients were all Various symptoms account for the largest proportion, and more than half of them. Conclusion :(1)The stage changes from mild cognitive impairment to dementia, and the age is gradually increasing. It can be seen that cognitive impairment is affected by age factors;(2)In the stages of MCI,VCI to AD,Va D,kidney deficiency syndrome, spleen deficiency syndrome, blood deficiency syndrome, and phlegm turbidity syndrome have obvious performance on cognitive impairment;(3)In the course of the onset of cognitive disorders, most symptoms coexist. |
