| Objective:In the real world environment,through the prospectie cohort study,observe the clinical effects of 120 HFMD patients with normal,summarizes the clinical medication rule,at the same time to the treatment of hand,foot and mouth disease of traditional Chinese medicine clinical technology guide(2012 edition)"in the pediatric clinical curative effect of TCM on guidelines,hand,foot and mouth disease(revised)2016 edition guidelines for the standard,evaluation of clinical treatment and guide conformity degree,for the application of the guide provides reference and revision.Methods:Collection of Department of Pediatrics,Affiliated Hospital of Shandong University of Traditional Chinese Medicine between January 2019 and December 2019,in accordance with the traditional Chinese medicine treatment of hand,foot and mouth disease clinical technology guide(2012 edition)"in the pediatric clinical curative effect of TCM on guidelines,hand,foot and mouth disease(revised)2016 edition(hereinafter referred to as the"guidelines")in normal children with hand,foot and mouth disease,according to the acceptance of drugs in children with cohort study methods are divided into proprietary Chinese medicine treatment group and Chinese medicine treatment group,through the observation with the signs and symptoms of integral before and after treatment,evaluate the effectiveness of the treatment and summarize clinical medication rule,and evaluate it with the guide of conformity degree.Result:1.After 5 days of treatment,the total effective rate was 95.24%in the proprietary Chinese medicine group and 96.49%in the traditionalChinese medicine group,respectively,with no significant difference(P>0.05).The total effective rate of the two groups was 96.83%and 98.25%respectively,and there was no significant difference after statistical analysis(P>0.05).In terms of improvement of symptoms and signs,there was no significant difference between the two groups in terms of rash/herpes,oral ulcer,fever,cough,run ting nose,pharyngeal pain,poor appetite,nausea and vomiting(P>0.05).In terms of oral ulcer and pharyngeal pain,the Chinese patent medicine group was significantly better than the Chinese traditional medicine group(P<0.05).2.In terms of drug use,the Chinese patent drugs with a high frequency of use are open throat sword(children)spray,qingqiao granules for children,pudilan xiaoyan oral liquid,kangxin fluid and lianhua qingwen granules;The main efficacy of proprietary Chinese medicine is to clear heat and detoxification,benefit the pharynx and reduce swelling,remove pathogenic factors,promote lung and cough.The top 20 Chinese herbs were forsythia,honeysuckle,roasted licorice root,agaricola,scutellaria baicalensis,mint,Fried burdock seed,combrosia chinensis,platycodon platycodon,acorus gramineus,bamboo leaf,radix isatidis,talc,bupleurum chinense,radix puerariae,radix puerariae,rhizoma ruticosa,and reed root.According to the classification of traditional Chinese medicine,the most commonly used antipyretic and antidotal drugs were followed by antidotes,clearing damp drugs and blood-cooling drugs.The research on Chinese medicinal properties showed that the properties of medicinal properties were mainly cold,followed by warm,cool and flat.The medicine taste is mainly bitter,xin,gan;Less salty,sour and astringent;The main meridians are lung meridians,followed by spleen meridians and heart meridians.3.In terms of conformance evaluation,the application coincidence rate of "diagnosis" in the guidelines is the highest(100%);The coincidence rate of the application of "syndrome differentiation" and "rule of treatment" was 91.67%.The coincidence rate of"prescriptions" was 89.48%.The coincidence rate of "Chinese patent medicine"was 86.95%.The clinical apparent rate was 82.5%.Conclusion:1.TCM treatment of common hand,foot and mouth disease is effective.2.In combination with the clinical law of drug use and the evaluation of the compliance degree of clinical treatment,the guide is applicable to the clinic and is a standard scheme for clinicians to usetraditional Chinese medicine to treat HFMD.3.Suggestions for future revision of the guide:(1)in terms of syndrome type,the syndrome of accumulation of heat in the heart and spleen is clearly identified as one of the common syndromes of hand-foot-mouth disease,and the corresponding treatment method is to clear heat and purge the spleen,purge fire and detoxify.(2)according to the frequency,effectiveness and safety of proprietary Chinese medicine in the clinical treatment of common hand foot and mouth disease,and based on the evidence-based evidence of children’s experience in drug use,the varieties of proprietary Chinese medicine should be adjusted appropriately,and the application of local drugs should be promoted at the same time;(3)in order to facilitate the mastery of the guide and its widespread application in clinical practice,especially to meet the use requirements of western doctors,the correlation between TCM syndromes and symptoms as well as the indications for the use of proprietary Chinese medicine can be further refined. |
