Dangguibuxuetang Combined With LOW Molecular Weight Heparin To Prevent Perioperative Deep Venous Thrombosis Of Lower Extremity In The Elderly With Femoral Trochanteric Fractures

Objective:To observe the efficacy of angelica buxue decoction combined with low molecular weight heparin in preventing deep vein thrombosis(DVT)in senile femoral trochanteric fractures during perioperative period,and to evaluate the effect of angelica buxue decoction in treating deep vein thrombosis in senile femoral trochanteric fractures.To provide a method and reference for the prevention and treatment of DVT after femoral trochanteric fracture in elderly patients.Methods:According to the inclusion criteria of the experiment,80 patients receiving surgical treatment for femoral trochanteric fracture in orthopedic trauma department of affiliated hospital of shandong university of traditional Chinese medicine from April 2019 to December 2019 were selected and divided into the experimental group and the control group according to the random number table,with 40 cases in each group.The control group received routine postoperative anticoagulant therapy,that is,5000IU of LMWH was used subcutaneously 12 hours after surgery for 5 consecutive days,and routine nursing of anticoagulant therapy was also given.The experimental group was given 200ml of angelica buxue decoction on the basis of the treatment of the control group.To observe and compare the two groups of patients in the preoperative and postoperative day one,three,five days of the lateral crus surrounds,VAS pain score,prothrombin time(PT),activated partial clotting enzyme live time(APTT),fibrinogen(FiB),D-dimer index changes,and double lower limbs venous doppler ultrasound for the formation of DVT.Results:The basic data(age and gender)of the experimental group and the control group were compared,and the difference between the two groups was not statistically significant.There were a total of 66 patients in the experimental group and control group,and 5 patients were diagnosed with DVT by bilateral lower limb doppler ultrasonography after surgery,with an incidence of 7.58%.In the control group,33 patients were diagnosed with DVT,3 of which had an incidence of 9.09%.In the experimental group 33,2 cases were diagnosed with DVT,the incidence of which was 6.06%.By chi-square test p>0.500,there was no significant difference between the two groups.On the third day after surgery,the crus circumference and VAS score of both groups were 0.239 and 0.394 respectively.PT p=0.042,APTT p=0.00,Fib p=0.00.P<0.05 of these three items had statistical differences.Plasma d-dimer p=0.083,p>.05 had no significant difference.On the fifth day after the operation,crus circumference of the affected side of the two groups was p=0.024,VAS score p=0.032,PT p=0.000,APTT p=0.000,Fib p=0.008,and plasma d-dimer p=0.028,all p<0.05,showing statistical differences.Conclusions:Dangguibuxuetang combined with LMWH can effectively prevent DVT after femoral trochanteric fracture in the elderly,and its clinical effect is similar to that of LMWH alone.However,combined application has advantages in improving the degree of crus swelling on the affected side of patients,which is worthy of clinical promotion.
