| Objective: To explore the feasibility of sentinel lymph node biopsy without using tracers.Methods: 47 patients with breast cancer(preoperative axillary lymph node clinical negative)who underwent breast cancer surgery in our hospital from March 2018 to March 2019 were selected.In the control group,23 patients underwent sentinel lymph node biopsy(SLNB)with indocyanine green(ICG)and methylene blue,and the lymph nodes were sentinel lymph nodes(SLN).22 patients in the experimental group underwent a transverse incision(about 2 cm)about 3 cm below the third fold of the axilla to remove the lymphatic adipose tissue 3-5 cm in diameter in the operative field,which was defined as low lymph node biopsy(LLNB),and the separated lymph nodes were defined as low lymph node(LLN).All patients underwent axillary lymph node dissection(ALND),and the obtained lymph nodes were axillary lymph nodes(ALN).LLN or SLN obtained during operation were sent to rapid frozen pathology and ALN to routine pathological examination.Results: There was no significant difference in the basic clinical data between the two groups(P > 0.05);the average number of lymph nodes detected in the experimental group was much higher than that in the control group(6.2 vs 3.6,P = 0.000);the detection rate in the experimental group was lower than that in the control group(95.5% vs 95.6%,P = 0.578),and there was no significant difference;the false negative rate in the experimental group was lower than that in SLNB(16.7% vs 50%,P = 0.260).There was no significant difference.Conclusion:The amount of data in this clinical comparative experiment is small,and there is obvious contingency.The false negative rate obtained is obviously deviated from the results of large data at home and abroad.However,LLNB is superior to ICG combined with methylene blue SLNB in detecting the number of lymph nodes.In other words,it is feasible to obtain a large number of sentinel lymph nodes by means of fixed anatomical location without using dyes and tracers,but the false negative rate and detection rate need to be further compared with large data to reach a conclusion. |
