Study On The Reliability Of HyCoSy And HSG For The Tubal Patency And Pelvic Adhesions

Objective To evaluate the diagnostic accuracy of HyCoSy and HSG in the diagnosis of tubal pathology among infertile patients.Methods A retrospective study in the first affiliated hospital of Hainan medical university was performed.Collect information of impatient and outpatient,select the patients who had done HSG or HyCoSy and be diagnosed as incomplete patency oviduct from January 1,2015 to December 31,2019.The patients also had done laparoscopy in our hospital.Respectively analysis the rustle of HSG and HyCoSy and laparoscopy.General tubal pathology,tubal occlusion,and peritubal adhesions detected at hysterosalpingography were compared with general tubal pathology,tubal occlusion,and peritubal adhesions detected at laparoscopy.Results(1)The study population comprised 122 infertile women,221 oviducts.The accuracy of HyCoSy in diagnosis of general tubal pathology is 48.4%,the sensitivity of 59.6%and specificity of 92.0%,the likelihood ratio of a positive test result of 87.7%and a negative test result of 59.0%while evaluating general tubal occlusion was determined.Compared with LC,χ~2=26.980,P<0.001)was statistically significant.When definition of peritubal adhesions,the accuracy was60.0%,the sensitivity and specificity were 22.1%and 93.4%and likelihood ratios87.7%and 59.0%respecitively.Compared with LC,(χ~2=11.177,P<0.001)was statistically significant.(2)The study population comprised 129 infertile women,255oviducts.The accuracy of HSG in diagnosis of general tubal pathology is 63.1%,the sensitivity of 60.7%and specificity of 89.2%,the likelihood ratio of a positive test result of 86.3%and a negative test result of 66.9%while evaluating general tubal occlusion was determined.Compared with LC,(χ~2=12.67,P<0.001)was statistically significant.When definition of peritubal adhesions,the accuracy was 55.4%,the sensitivity and specificity were 39.2%and 85.8%and likelihood ratios 55.4%and75.9%respecitively.Compared with LC,(χ~2=9.78,P<0.001)was statistically significant.Conclusions Hysterosalpingography is more accurate in the diagnosis of general tubal occlusion.The diagnostic performance of Hysterosalpingography is more poor in the diagnosis of peritubal adhesions.
