Objective To analyze the incidence of cardiovascular and cerebrovascular diseases and the influencing factors in a prospective cohort,and to study the dose-response relationship between the influencing factors and the risk of CVD,as well as the interaction between the influencing factors,so as to provide scientific basis for the prevention and treatment of CVD in the population.Methods In this study,based on Jinchang cohort,47998 subjects were selected from the baseline of the cohort.The epidemiological data of the subjects were obtained by face-to-face interview.Each subject received a physical examination and obtained its physiological and biochemical indexes.The demographic characteristics,the prevalence of CVD and the scores of physiological and biochemical indexes were analyzed by descriptive research methods The incidence rate and risk of CVD were analyzed by cohort study.The relative risk and the 95% confidence interval were calculated.The influence factors and heart and brain blood were analyzed by the restricted spline method,and the relative risk and 95% confidence interval were calculated.The dose-response relationship between the risk of CVD was analyzed by the multiplicative interaction model.Results 1 In the male population,female population and the general population,the incidence of cardiovascular and cerebrovascular diseases in TG,TC,hdl-c,ldl-c abnormal people and those who quit smoking,drinking,abstaining from alcohol,drinking and drinking tea were higher than those in the normal population,and the incidence of female abnormal people was higher than that in the male population.2 After controlling age,education level,income level and occupation,the risk of CVD was 4.37,1.74,2.22,1.70,1.41,1.37,1.18 and 1.31,1.50,1.31 times higher in those with abnormal blood pressure,blood glucose,BMI,TG,TC,HDL-C,LDL-C,smoking,drinking and abstinence,respectively.These factors are the influencing factors of cardiovascular and cerebrovascular diseases,among which blood pressure is the most obvious.Among the three cardiovascular and cerebrovascular diseases,blood pressure,blood glucose,BMI,TG,TC,HDL-C and LDL-C have the most obvious effects on hypertension.3 BMI,blood pressure,blood glucose,TG,TC have a positive dose-response relationship with the risk of cardiovascular and cerebrovascular diseases.With the increase of BMI,blood pressure,blood glucose,TG,TC,the risk of disease also increases;HDL-C has a negative dose-response relationship with the risk of cardiovascular and cerebrovascular diseases,and with the increase of HDL-C,the risk of disease decreases.4 There is a multiplicative interaction between blood pressure abnormality and blood lipid,blood glucose abnormality,overweight,smoking and drinking.The joint effect of the two factors can increase the risk of cardiovascular and cerebrovascular diseases,making it higher than that of normal people and people suffering from a metabolic disease.Conclusion 1 The incidence rate and risk of cardiovascular and cerebrovascular diseases in Jinchang cohort are closely related to metabolic diseases and biochemical indicators.High blood pressure,overweight and obesity are the main influencing factors of cardiovascular and cerebrovascular diseases in a cohort.At the same time,TG,TC,HDL-C also had significant effects on cardiovascular and cerebrovascular diseases in a cohort.2 There is a positive correlation between BMI,blood pressure and the risk of cardiovascular and cerebrovascular diseases,and a negative correlation between HDL-C and the risk of cardiovascular and cerebrovascular diseases.And the combination of two metabolic diseases will increase the risk of cardiovascular and cerebrovascular diseases.3 In Jinchang cohort,reducing blood pressure and controlling body weight should be the focus of health intervention.For patients with dyslipidemia,regular physical examination should be carried out to prevent cardiovascular and cerebrovascular diseases in time.We should pay close attention to female abnormal population and prevent cardiovascular and cerebrovascular diseases.
