ObjectiveThe Laser speckle contrast analysis technology is used to monitor the blood flow perfusion in the ocular and cheek areas of patients with peripheral facial paralysis who received acupuncture treatment in different disease stages;and the facial nerve function is scored.On this basis,the dynamic changes of blood flow asymmetry value(Pr value)and facial nerve function score are studied,and the correlation between blood flow asymmetry value(Pr value)and facial nerve function score is analyzed,and then the acupuncture’s curative effect of the disease and the difference in the effect of receiving acupuncture in different stages is objectively observed and evaluated.MethodsAccording to whether the onset time of the patients at the first diagnosis of acupuncture is in the acute phase(within 7 days),80 patients with peripheral facial paralysis who meet the inclusion criteria are collected.The acute phase group and the non-acute phase group each has 40 cases.Acupuncture is the mainly treatment program,30min a time,3 times a week,1 week for 1 course of treatment,a total of 3 courses.The laser speckle contrast analysis technique is used to monitor the facial blood perfusion,and the patients’ facial nerve function is scored before the acupuncture treatment on the 1st day of the first diagnosis,the 7th day,the 14th day,and the 21st day.The facial blood flow asymmetry value(Pr value)before the acupuncture treatment on the 1st day of the first diagnosis,the 7th day,the 14th day,and the 21st day and the corresponding facial nerve function score are then analyzed with statistics.Results1.Asymmetry of blood flow in the eye area(Pr value)The change trends of the two groups are not completely consistent,but the overall trend is a downward trend.The Pr value on the 7th day of the acute phase group was higher than the Pr value on the 1st day of the first diagnosis,and the Pr values at the other treatment time points are lower than before,the differences are statistically significant(P<0.01).In the non-acute phase group,there is no statistically significant difference in the Pr value between the first day and the 7th day of the first diagnosis(P>0.05);after that,the Pr value gradually decreases,and the difference between the time points is statistically significant(P<0.05).The Pr value compared between the two groups comes out with a statistically significant difference in the Pr value on the 7th day-the acute phase group is higher than the non-acute phase group(P<0.05).The Pr values between the two groups at the other time points have less difference and there is no statistical significance(P>0.05).2.Asymmetric value of blood flow in cheek area(Pr value)The trends of the two groups are not completely consistent.The Pr value on the 7th day of the acute phase group is higher than the 1st day of the first diagnosis,the difference is statistically significant(P<0.05),and there is no statistically significant difference on the 14th,21st day and the 1st day of the first diagnosis(P>0.05);the Pr values on the 14th and 21st days are lower than those on the 7th day,and the differences are statistically significant(both P<0.01);the difference between the 14th and 21st days is not statistically significant(P>0.05).There is no statistically significant difference in Pr value between the non-acute phase groups at each time point(P>0.05).There is no statistically significant difference in Pr value between the two groups at each time point(P>0.05).3.Facial nerve function scoreThe facial nerve function scores of both groups show an upward trend as a whole.There is no statistically significant difference between the score on the 7th day and the 1st day of the first diagnosis in the acute phase group(P>0.05);after that,the scores increased at each time,and the change range was large,the difference is statistically significant(P<0.01).In the non-acute phase group,the score on the 7th day is compared with the first day of the first diagnosis,and the difference is not statistically significant(P>0.05);after that,the score increased at each time,and the difference is statistically significant(P<0.05).On the 7th day,the score in the acute phase group is lower than that in the non-acute phase group,and the difference is statistically significant(P<0.05);the scores between the two groups at the other time points are not statistically significant(P>0.05).4.Correlation between blood flow asymmetry value(Pr value)and facial nerve function scoreBoth two groups’ facial nerve function scores are negatively correlated with the Pr value of the eye area at each time point.On the 1st,7th and 14th days of the acute phase group,the correlation between the scores and the Pr value at the corresponding time point is statistically significant(all P<0.01);on the 1st,14th and 21st days of the non-acute phase groups,the correlation between the scores and the Pr value at the corresponding time point is statistically significant(P<0.01,0.01,0.05,respectively).Both two groups’ facial nerve function scores are negatively correlated with the Pr value of the cheek area at each time point.The correlation on the 14th day of the acute phase group is statistically significant(P<0.05),and the correlations at other time points are not statistically significant(P>0.05).5.EfficacyThere is no statistically significant difference between the two groups at the end of the 3 treatment courses(P>0.05).In the acute phase group,the cure rate is 34.21%,the apparent efficiency rate is 50%,the effective rate is 15.79%.And in the non-acute phase group,the cure rate is 27.03%,the apparent efficiency rate is 43.24%,and the effective rate is 29.73%.After 3 months of follow-up,the difference between the two groups is statistically significant(P<0.05).In the acute phase group,the recovery rate is 63.16%,the apparent efficiency rate is 23.68%,and the effective rate is 13.16%.And in the non-acute phase group,the cure rate is 37.84%,the apparent efficiency rate is 34.21%,and the effective rate is 27.03%.The recovery rate in the acute phase group is higher than that in the non-acute phase group(P<0.05).And comparing the short-term efficacy(at the end of 3 courses)with the long-term efficacy(follow-up after 3 months),the long-term efficacy of the two groups are better than the short-term efficacy(P<0.05).Conclusion:During the acupuncture treatment,as the number of acupuncture treatments increasing,the blood flow asymmetry value(Pr value)of the affected eye area gradually decrease to near“1”in the acute phase group and the non-acute phase group.The overall Pr value of the cheek area dose not change significantly.The facial nerve function scores gradually increase,suggesting that the facial nerve function is gradually improved.The facial nerve function score has a significant negative correlation with the Pr value in the eye area,suggesting that the Pr value in the eye area can be used to evaluate the facial nerve function of peripheral facial paralysis and the possibility of observing and evaluating the efficacy of facial paralysis.There is no significant difference in the short-term efficacy between the two groups,but from the perspective of long-term efficacy,the acute phase group is better than the non-acute phase group,suggesting that acupuncture treatment at the onset has a definite effect,and the long-term effect is better than receive acupuncture treatment at the non-acute phase.Taking acupuncture and moxibustion treatment in the acute phase of peripheral facial paralysis can achieve better therapeutic effects.
