ObjectiveThis research is to observe the change of blood perfusion in the process of acupuncture treatment of intractable facial paralysis by using Laser Speckle Blood Flow Imaging,combined eleetromyogram(EMG)and clinical symptoms and signs to clear the treatment effect.According to Chinese medicine theory to provide new ideas for clinical treatment of peripheral facial paralysis as well as open new methods to evaluating clinical treatment.MethodsWe collected 70 patients with refractory peripheral facial paralysis who go to Shenzhen Tratreatmentditional Chinese Medicine Hospital to look for help.Then divide them into 3 groups dialectically.Each group will be treated 3 days a week(once a day)and rest for the every other day.The total period of treatment is one month,and it will be stopped if the patient gets well within a month.We observed facial blood perfusion and electrophysiology before and after the treatment,evaluated and recorded the clinical efficacy of before and after a month by using score tables of facial nerve function.We evaluated the correlation and accuracy of the three indexes,and used the SPSS 22 version software package to analyzed the data.Results1.There were no statistically significant difference(P>0.05)among every one when comparing their sex,age,electrophysiology and facial blood perfusion before treatment.2.In the process of,we compared the perfusion ratio(Pr)of facial blood perfusion of three groups of patients ten minutes before and after the acupuncture,found that P<0.05 in the buccal and mandibular region,which indicated that there were significant difference between before and after treatment.3.After one month of acupuncture treatment,the facial nerve function score of the three groups P<0.05,showed that they all had a significant difference when compared with a month ago.But the results reversed when compared between respectively groups.After a month of acupuncture treatment,the patients ’ amplitude of orbicularis oris muscle had a significant difference when compared with a month ago,its P<0.05,As for the Pr of facial blood perfusion,the circumstance was the opposite.Compared with a month ago,there showed significant different among the whole groups of the patients,P>0.05,except the blood perfusion of ocular region in qi deficiency and blood stasis group.4.After a month of acupuncture treatment to intractable facial paralysis,we made a correlation analysis of the Pr of facial nerve function score,the amplitude of orbicularis oculi muscle and orbicularis oris muscle as well as the blood perfusion of three facial areas.We found that facial nerve function score had a significant correlation to the amplitude of orbicularis oculi muscle and orbicularis oris muscle as well as the blood perfusion of buccal and ocular region,but had not to the blood perfusion of mandibular.5.After one month of acupuncture treatment in both groups,the cure rate of the two groups was compared,P<0.01,indicating that there was a significant difference in the cure rate between the two groups.In patients who recovered within one month after acupuncture treatment,the average number of treatment times in the two groups was P<0.01,indicating that there was a significant difference between the two groups in the average number of treatments.6.During the whole treating process,there was no obvious adverse reactions occurred among all the patients,so the safety level can be Level 1.ConlusionThrough the above studies,the writer believes that acupuncture can be used to treat refractory facial paralysis.It can release the clinical symptoms and is efficacy.As for the aspect of evaluation criteria,the electrophysiological index as the commonly used facial nerve function evaluation standard has the evaluation credibility.The technique of laser speckle can immediately reflect the effect of acupuncture to turn on the blood perfusion,and can indicate that acupuncture can improve blood circulation,do well to the recovery of facial nerve objectively.It can be a measure to evaluate and has certain credibility to evaluating the effect of refractory facial paralysis.
