| Objective It aims to analyze the malignant tumor mortality, distribution characteristics and trend of mortality over time in residents of seven death cause monitoring sites in Guangxi from 2008 to 2016,so as to further understand the current status and changing trends of deaths due to malignant tumors among residents in Guangxi, providing scientific basis for formulating measures and strategies for the prevention and treatment of malignant tumors. Methods Data on deaths from malignant tumors in seven death cause monitoring sites in Guangxi from 2008 to 2016 in the death cause registration report information system was collected and classified and coded according to the International Classification of Diseases,Tenth Revision(ICD-10)to calculate the crude death rate, standardized mortality rate, proportion, rank order of death cause, etc.;statistical analysis was performed using SPSS20.0 software;the mortality rate was compared using thec~2 test;the trend of the mortality rate with age was analyzed using the trendc~2 test, and the significance levelα=0.05.The time trend of malignant tumor mortality data was analyzed by using Jointpoint regression model. Results 1.From 2008 to 2016,among the permanent residents in seven death cause monitoring sites in Guangxi, the deaths due to malignant tumors was 38409,the crude death rate was 132.00/100 000,and the age-standardized rate(Chinese population)was 109.67/100 000.The crude death rate of malignant tumors among males was 175.13/100 000,and the age-standardized rate(Chinese population)was 147.73/100 000,which were significantly higher than those among females(the crude death rate was 84.64/100 000,and the age-standardized rate by Chinese standard population was 68.74/100 000).The mortality rate due to malignant tumors increased with age(trendc~2=68665.61,P<0.001).Among and over the 55-year-old population, the proportion of deaths due to malignant tumors accounted for 69.80%of the total.2.In terms of ethnic distribution, the Han nationality had the most deaths, accounting for 80.27%of the total deaths due to malignant tumors. In the occupational distribution, farmers had the most deaths, accounting for 69.12%of the total deaths due to malignant tumors, and the number of deaths due to malignant tumors decreased with the higher education.3.Residents in Binyang County had the highest mortality due to malignant tumors, with the crude mortality rate of 163.29/100 000,and Xiufeng District had the least, with the crude mortality rate of 92.32/100 000.The crude death rate of malignant tumors in rural areas(141.87/100 000)was higher than that in cities(108.82/100 000),and the difference was statistically significant(P<0.05).The mortality rate of malignant tumors had increased with the years. The crude mortality rate increased from 99.47/100 000 to 132.00/100 000,and the age-standardized rate(Chinese population)increased from 94.32/100 000 to105.10/100 000.4.In terms of the rank order of death cause, the top ten were liver cancer, lung cancer, gastric carcinoma, colorectal cancer, nasopharyngeal cancer, brain malignant tumor, leukemia, esophagus cancer, breast cancer, bone and articular cartilage malignant tumor, with the death toll accounting for 86.36%of the total deaths. As for the rank order of death cause among different age groups, leukemia was the first in the group of 0-14 years old,and liver cancer was the first in the group of 15-44 years old among male and female.Respectively, liver cancer and lung cancer were the first in the male and female group of 45-64years old,and lung cancer were the first in the group of 65 years old and above.Liver cancer was the leading cause of death from malignant tumors in rural areas, and lung cancer were leading in cities.5.Joinpoint model analysis revealed that the crude mortality rate showed a continuous upward trend, with the APC of 4.64%,and the age-standardized rate(Chinese population)of malignant tumors trend had no statistical significance. The crude death rate and the age-standardized rate(Chinese population)of urban residents showed a continuous upward trend. And the crude death rate and the age-standardized rate(Chinese population)of rural residents showed a steady trend after rising(the turning point was in 2011).Mortality rates due to lung cancer, colorectal cancer, and breast cancer all continued to rise, and the rising trend of the age-standardized rate(Chinese population)was less than that of the crude death rate. The crude death rate due to liver cancer showed a continuous upward trend, and the age-standardized rate(Chinese population)was not statistically significant. Both the crude mortality rate and age-standardized rate(Chinese population)trend of gastric carcinoma and nasopharyngeal carcinoma had no statistical significance. Conclusions 1.From 2008 to 2016,the level of death due to malignant tumors of seven death cause monitoring sites in Guangxi was still at a relatively high level in China.Liver cancer, lung cancer, gastric carcinoma, colorectal cancer and nasopharyngeal cancer were the main causes of death. And it has shown different mortality level and death causes among the population from different region, gender and age groups.2.The crude mortality rate of malignant tumors continued to rise, and mortality levels remained relatively stable after standardization. The mortality of malignant tumors is on the rise in urban areas, while it is on the rise first and then on to a stable trend in rural areas. The mortality rate among the population from lung cancer, colorectal cancer and female breast cancer has continued to rise. |
