| ObjectiveTinnitus is an illusory feeling produced without auditory stimulation.Different types of tinnitus are different in pathophysiology and response to specific treatments.At present,the efficacy of many treatment methods is not clear and the effects are not uniform,but the factors that affect the treatment effect are still unknown.Therefore,in future clinical trials to treat tinnitus,it is important to detennine the type of tinnitus and analyze its characteristics.Therefore,the purpose of this part of the study is to analyze the effects of different tinnitus characteristics and factors on the efficacy of refined acoustic therapy.In addition,this study found that the tinnitus loci "drift" in some patients after refined acoustic treatment and further explored the value of frequency-shifted tinnitus sites for the guidance of refined acoustic therapy in tinnitus patients.MethodsA retrospective analysis of 156 patients(216 ears)whose first complaint was an ear,nose and throat clinic outpatient visit in our hospital was the general information,conventional pure tone audiometry,tinnitus refinement test matching status,and THI scale score.According to the pure audiometry results,the subjects were divided into normal hearing tinnitus group and hearing loss tinnitus group.All tinnitus patients were tested for refined tinnitus sites and personalized sound therapy,and the tinnitus disability scale(Tinnitus Handicap Inventory,before treatment,1 week,1 month,and 3 months after treatment)was used.THI)to evaluate clinical efficacy.Results1.156 cases(216 ears)of tinnitus patients,the total effective rate of acoustic therapy was 48.08%;2.There is no statistically significant difference in the effect of fine acoustic therapy between normal hearing group and hearing loss group(χ2=0.966,P=0.326);3.There is a statistically significant difference in the efficacy of tinnitus sites at different frequencies between the normal hearing group and the hearing loss group(respectively χ2= 9.336,P = 0.009 and χ2=27.971,P=0.000).4.In the normal hearing group,the tinnitus site matching group and the unmatched group had a statistically significant difference in efficacy after 3 months of sound therapy(χ2=6.529,P=0.038).The matched group had better efficacy than the unmatched group and no significant difference in hearing loss group(χ2=2.420,P=0.298)5.After precision sound treatment,there were 26 cases(16.67%)of tinnitus frequency drifting;14 cases in normal hearing group and 12 cases in hearing loss group;there was no statistics on the change rate of tinnitus locus in two groups Academic difference(P=0.203)6.In all patients with frequency shifted(26 cases),the tinnitus site moved from a higher frequency to a lower frequency,and the loudness was lower than before.The average frequency of tinnitus sites before frequency shift was 4527Hz,and the loudness was 30dB.,The average frequency of the site is 4381Hz,28dB;7.The results of multivariate logistic regression analysis found that high-frequency tinnitus sites and frequency shifted are beneficial factors that affect the efficacy of patients.ConclusionPrecision sound therapy has a certain effect on patients with tinnitus regardless of hearing loss or normal hearing.After treatment,tinnitus patients may experience a"drift" phenomenon,and the occurrence of frequency-shift tinnitus corresponds to a better treatment effect.The key to the treatment is to refine the tinnitus site and personalize the "sound formula" treatment. |
