| Objective:1.To understand the epidemiological characteristics of acute cerebral apoplexy in liuzhou;2.To understand the risk factors of acute str oke in liuzhou city. Methods:Workers in liuzhou city hospital between January 2017 and December 2019 were hospitalized patients with acute cerebral apoplexy were retrospectively analyzed, collected 4432 cases of patients with acute cerebral apoplexy, including 2472 cases of patients with cerebral infarction, cerebral hemorrhage patient 1448 cases,512 cases of patients with subarachnoid hemorrhage, the collection of the medical health physical examination in our department at the same period of 400 cases of cerebrovascular disease patients as control group. The study included the patient’s gender, age, time of onset, time from onset to admission, history of hypertension, history of diabetes, history of hyperlipidemia, history of atrial fibrillation, history of smoking and history of alcohol consumption. To explore the epidemiological characteristics of acute stroke;Risk factors of stroke were analyzed by Logistic regression analysis.Results:1.A total of 4432 patients with acute cerebral apoplexy were investigated, including 2472 patients with cerebral infarction(55.78%),1448 patients with cerebral hemorrhage(32.67%),and 512 patients with subarachnoid hemorrhage(11.55%).Among the patients with cerebral infarction, there were 1725 males(69.78%)and 747 females(30.22%).The number of males was higher than that of females, and the ratio of male to female was 2.31:1.Among the patients with cerebral hemorrhage, there were 971 males(67.06%)and 477 females(32.94%).Among the patients with subarachnoid hemorrhage, there were 204males(39.84%)and 308 females(60.16%),and the number of females was higher than that of males, and the ratio of male to female was 1:1.51.Among the three groups, there was a statistical difference in male and female composition ratio(X~2=170.585,P<0.01).2.The age of onset of cerebral infarction was mainly between 51 an d 80 years old,and the incidence of male and female patients peaked at61 to 70 years old.The number of males in each age group was more tha n that of females. There were statistical differences between male and fema le patients in all age groups(X~2=103.337,P<0.01).The age of onset of intr acerebral hemorrhage was mainly between 51 and 80 years old.The incid ence peaked at 51 to 60 years old for males and 61 to 70 years old for females. There were more males than females in each age group, and the i ncidence of disease increased gradually with the growth of age. There wa s a statistical difference between male and female patients in each age gr oup(X~2=24.832,P<0.01).The age of onset of subarachnoid hemorrhage is mainly between 51 and 70 years old.The incidence increases with age;The re were statistical differences between male and female patients in all age groups(X~2=47.7,P<0.01).There were statistically significant differences in the incidence of cerebral infarction, cerebral hemorrhage and subarachnoid hemorrhage among the three groups(X~2=161.256,P<0.01).3.The incidence of cerebral infarction was high in winter, with 780 c ases(31.55%).There was no statistical difference between male and femal e patients in each season(X~2=1.517,P>0.05);The incidence of cerebral he morrhage was high in winter, with 463 cases(31.98%).There was no stat istical difference between male and female patients in each season(X~2=1.566,P>0.05);The incidence of subarachnoid hemorrhage was high in winter, with 179 cases(34.96%).There was no statistical difference between male and female patients in each season(X~2=0.783,P>0.05).There was no statis tical difference between the three groups of patients with cerebral infarctio n,cerebral hemorrhage and subarachnoid hemorrhage in each season(X~2=3.178.P>0.05).4.The time from onset to admission of patients with cerebral infarctio n was≤3 hours in 619 cases(25.04%),3 to 6 hours in 491 cases(19.86%),and more than 6 hours in 1362 cases(55.1%).There was no statistic al difference between male and female patients from onset to admission(X~2=2.949,P>0.05).The time from onset to admission of patients with intra cerebral hemorrhage≤3 hours was 472(32.60%),3 to 6 hours was 307(21.20%),and 669(46.2%)were more than 6 hours. There was no statistic al difference between male and female patients from onset to admission(X~2=3.726,P>0.05).The time from onset to admission of patients with sub arachnoid hemorrhage≤3 hours was 151(29.49%),3 to 6 hours was 107(20.90%),and 254(49.61%)were more than 6 hours. There were statistical differences between male and female patients from onset to admission(X~2=9.916,P<0.05).There were statistically significant differences between t he three groups of patients with cerebral infarction, cerebral hemorrhage a nd subarachnoid hemorrhage from onset to admission(X~2=46.221,P<0.01).5.Patients with cerebral infarction, cerebral hemorrhage and subarachn oid hemorrhage had the highest prevalence of hypertension.6.Logistic regression analysis showed that male, aging, hypertension, diabetes, hyperlipidemia, atrial fibrillation and smoking were the risk factors for cerebral infarction.Male, aging, hypertension and hyperlipidemia are the risk factors for cerebral hemorrhage. The risk factors of subarachnoid hemorrhage are female, aging, hypertension and hyperlipidemia.Conclusion:1.The incidence of acute stroke in liuzhou city was dominated by cerebral infarction. The incidence of cerebral infarction and cerebral hemorrhage was more in males than females, and the incidence of subarachnoid hemorrhage was more in females than males;2.With the growth of age,the incidence of stroke increases gradually;3.The incidence of cerebral infarction, cerebral hemorrhage and subara chnoid hemorrhage is high in winter;4.Many stroke patients had a long time from onset to admission and missed the optimal treatment time;5.Patients with cerebral infarction, cerebral hemorrhage and subarachn oid hemorrhage had the highest prevalence of hypertension;6.Male,age,hypertension,diabetes,hyperlipidemia,atrial fibrillation and smoking are the risk factors for cerebral infarction.Male, aging, hypertension and hyperlipidemia are the risk factors for cerebral hemorrhage. The risk factors of subarachnoid hemorrhage are female, aging, hypertension and hyperlipidemia. |
