Clinical Study Of Double Contrast-enhanced Ultrasonography In The Diagnosis Of Benign And Malignant Gastric Tumors

Objective: To explore the clinical value of Double Contrast-Enhanced Ultrasonography(DCEUS)in the diagnosis of benign and malignant gastric tumors and preoperative evaluation of TNM staging diagnosis of gastric cancer.Methods: 89 patients with gastric tumors in our hospital from August 2017 to March 2020 were selected as the research object.All patients underwent DCEUS(oral gastrointestinal ultrasound contrast and intravenous ultrasound contrast).the results of DCEUS examination were comparative with postoperative pathological results.Then,the pathologically confirmed gastric cancer patients’ preoperative staging results and clinical diagnosis results were compared and analyzed.Applying Kappa analysis method to evaluate the consistency of this method.Results: 1.89 patients with gastric tumors(a total of 89 tumors)confirmed by pathology,there were 29 cases of benign tumors,60 cases of malignant tumors.56 cases of gastric cancer and 4 cases of high-risk gastrointestinal stromal tumor in patients with malignant tumors.Postoperative clinical stage diagnosis of 56 patients with gastric cancer,there were 6 cases in T1 stage,12 cases in T2 stage,23 cases in T3 stage,15 cases in T4 stage;35 cases in N0 stage,14 cases in N1 stage,2 cases in N2 stage,5cases in N3 stage;50 cases in M0 stage,6 cases in M1 stage.2.The sensitivity,specificity,positive predictive value,and negative predictive value of DCEUS for the diagnosis of benign and malignant gastric tumors were96.67%,89.66%,95.08%,92.86%,respectively.The kappa values were consistent with pathological results were 0.871(P=0.000).3.The total coincidence rates of DCEUS for 56 cases of gastric cancer patients for the preoperative TNM staging were 83.93%(47/56),76.79%(43/56),98.21%(55/56),respectively.The kappa values consistent with the clinical staging diagnosis results were 0.770(P=0.000),0.525(P=0.000),0.899(P=0.000),respectively.Conclusion: DCEUS technique has a high diagnostic value in the differential diagnosis of benign and malignant gastric tumors and the preoperative TNM staging diagnosis of gastric cancer.It can be used as a repeatable and noninvasive method to provide more diagnosis information for clinicl.
