Objective:Evaluate the effect of radiofrequency surgery of suprascapular nerve pulse on severe scapulohumeral periarthritis,and provide reference for clinical treatment of severe scapulohumeral periarthritis.Methods:1.Choose 60 cases of severe scapulohumeral periarthritis treated in the Ya nbian hospital pain department from March 2018 to September 2019,and are r andomly divided into group A(pulse radiofrequency group,n=30)and group B(nerve block group,n=30).Patients in group A received pulse radiofrequency o f suprascapular nerve,while patients in group B received suprascapular nerve block.2.Treatment method:Group A:After local anesthesia with 1%lidocaine,insert a radiofrequency trocar at the needle insertion point,and then perform radiofrequency treatment after the needle reaches the suprascapular nerve(parameters:120s,2Hz,42℃,32v),treatment time 120s,after the treatment,inject 2ml of analgesic fluid at each tenderness point around the shoulder.Group B:The positioning of the surface of the suprascapular nerve body and the method of needle insertion are the same as in group A.A 7-gauge 10cm needle is inserted into the skin until the incision on the scapula and 10ml of analgesic fluid is injected.Both groups of patients were supplemented with pain point injections and combined with passive functional exercise.They were treated once a week for a total of five treatments.3.All patients were followed up for 6 months,comparing and analyzing the shoulder VAS(visual analog scale)score and Constant-Murley shoulder joint of the two groups of patients before treatment,1 week,1 month,3 months and 6 months after treatment Functional score and total effectiveness,use the scoring method to evaluate the total effectiveness,and observe whether there are complications.Results:1.VAS score:Compared with before treatment,the two groups of patients had decreased the VAS score of shoulder joints at 1 week,1 month,3 months and 6 months after treatment.The average VAS score of group A was lower than group B,all have statistical differences(P<0.05).2.Constant-Murley shoulder function score:Compared with before treatment,the Constant-Murley scores of the two groups of patients were increased to different degrees at 1 week,1 month,3 months,and 6 months after treatment.Group A VAS The average score was higher than that in group B,and all had statistical differences(P<0.05).3.The overall effectiveness of the two groups of patients was calculated by chi-square test is 90.0%and 66.7%.The overall effectiveness of the two groups of patients was statistically different(P<0.05).4.All patients had no adverse reactions such as nerve injury,infection,pneumothorax,hematoma,and drug allergy.Conclusion:1.Pulse radiofrequency surgery and nerve block surgery of suprascapular nerve can effectively treat severe scapulohumeral periarthritis.2.The pulse radiofrequency surgery of suprascapular nerve combined with pain point injection around the shoulder,and cooperated with passive functional exercise,is more effective than nerve block surgery in the treatment of severe scapulohumeral periarthritis.
