Objective:To select patients with primary nephrotic syndrome after renal biopsy,to determine the level of serum HIF-1α combined with laboratory indicators,to explore the serum HIF-la analysis of levels and correlations in patients with different pathological types of nephrotic syndrome.Methods:A total of 52 patients were selected patients with primary nephrotic syndrome after renal biopsy and complete clinical data.Divided into 24 cases of membranous nephropathy group,6 cases of minimally diseased nephropathy group,13 cases of IgA nephropathy group,9 cases of mesangial proliferative glomerulonephritis group and 52 cases of healthy physical examination as normal control group.ELISA method was used to detect the expression level of HIF-1α in serum.All data were analyzed using SPSS26.0 statistical software.One-way analysis of variance was used for the measurement data following the normal distribution,rank sum test was used for the skewed distribution,and linear correlation or rank correlation and linear regression were used for the correlation.Results:1.The levels of serum HIF-1α in the membranous nephropathy group(25.43±5.46 pg/ml),minor degenerative nephropathy group(25.71±3.21 pg/ml),IgA nephropathy group(24.40±4.88 pg/ml)and the mesangial proliferative glomerulonephritis(23.62±5.42 pg/ml)were higher than those in the normal control group(5.17±1.26 pg/ml),P value<0.05,The difference was statistically significant.2.The levels of serum HIF-1α were positively correlated with age,serum white blood cell,red blood cell,serum creatinine,cystatin C,total cholesterol,triglyceride,low-density lipoprotein,IgA,urinary protein quantification urinary,red blood cell,(correlation coefficients were 0.352,0.349,0.493,0.730,0.260,0.428,0.400,0.698,0.197,0.696,0.725,P value<0.05).They were negatively correlated with glomerular filtration rate,calcium,albumin,high-density lipoprotein,IgG and C3(correlation coefficients were-0.285,-0.618,-0.706,-0.424,-0.437,-0.238,P value<0.05).3.Linear regression analysis showed that age,phosphorus,cystatin C,albumin,total cholesterol,triglyceride,high-density lipoprotein,low-density lipoprotein,IgA were correlated with the levels of serum HIF-1α,and the regression coefficients were 0.090,-7.384,4.477,-0.245,-1.404,1.377,-1.578,2.828,1.606(P value<0.05),respectively.Cystatin C was still correlated with serum HIF-1 levels after adjusting for age;urinary protein quantification,glomerular filtration rate,serum creatinine,calcium,albumin;serum white blood cells,red blood cells,urinary red blood cells;IgA,IgG,C3;total cholesterol,triglyceride,high-density Lipoprotein,and low-density Lipoprotein,the regression coefficients were 13.848,12.135,8.634,8.415,8.571,7.951(P value<0.05).Conclusion:1.Patients with nephrotic syndrome generally have different degrees of hypoxia,but there was no significant difference in HIF-la levels between different pathological types.2.The increase of serum HIF-1α is related to the production of urinary protein.3.The increase of serum HIF-1α in patients with nephrotic syndrome is related to the deterioration of renal function.4.Serum HIF-1α level is correlated with cystatin C.
