ObjectiveTo evaluate the changes of left ventricular systolic function in patients with pregnancy induced hypertension(HDCP)by measuring the longitudinal,radial and circumferential strain of left ventricular systolic function before and after delivery with 3D-STI(Three-dimensional Speckle tracking imaging),and to evaluate the value of this method in the assessment of left ventricular systolic capacity in patients with HDCP.MethodsFrom October 1,2017 to October 31,2019,42 cases of pregnancy induced hypertension in the First Affiliated Hospital of Jinzhou Medical University and Jinzhou maternity and infant hospital were collected as the case group(HDCP group),42 cases of normal pregnancy and childbearing age pregnant women in the outpatient department and inpatient department of the two hospitals as the normal pregnancy group,and 42 cases of normal pregnancy and childbearing age pregnant women in the outpatient department of the First Affiliated Hospital of Jinzhou Medical University as the normal body group 45 non pregnant healthy women were selected as the control group.LVDD,LVDS,LVEDV,LVESV,EF,e,a and E/A of the three groups were measured by conventional left ventricular echocardiography.3d-sti was used to measure LVGLS and radial left ventricular systole Phase peak strain(LVGRS),left ventricular global systolic peak strain(LVGCS)and left ventricular global area systolic peak strain(LVGAS).Three months after delivery,two groups of pregnant women were reexamined by conventional echocardiography and 3d-sti technology.SPSS 22.0 statistical software was used for data processing,measurement data was described by(x±s)independent sample t-test,paired t-test and variance analysis were used for statistical inference,LSD method was used for comparison between the two,and the difference was statistically significant(P<0.05).Results(1)General situation of HDCP group,normal pregnancy group and control group:there was no significant difference in age,height,weight,gestational week and heart rate between HDCP group and normal pregnancy group,and there was no significant difference in age and height between HDCP group and control group,but the weight and heart rate were higher than that of control group.The mean values of systolic and diastolic blood pressure in HDCP group and normal pregnancy group were significantly higher than those in the control group.(2)LVDD,LVDS,LVEDV,LVESV,LVEF,e peak,a peak and E/a value of HDCP group,normal pregnancy group and control group before delivery were statistically significant(P<0.05),among which LVDD,LVDS,LVEDV,LVESV were HDCP group>normal pregnancy group>control group,LVEF,E/A value were HDCP group<normal pregnancy group<control group(P<0.05 for both comparisons);e peak The peak value of a and HDCP was only statistically significant(P<0.05).LVDD,LVDS,LVEDV,LVESV,LVEF,e peak and E/A value of HDCP group,normal pregnancy group and control group were statistically significant(P<0.05),LVDD,LVDS,LVEDV,LVESV,LVEF,e peak and E/A value were There was significant difference between HDCP group and normal pregnancy group,HDCP group and control group(P<0.05).(3)There were statistically significant differences in LVDD,LVDS,LVEDV,LVESV,LVEF,e peak and E/A value before and after delivery in HDCP group(P<0.05),among which the values of LVDD,LVDS,LVEDV and LVESV decreased,while the values of LVEF,e peak and E/a increased.There were significant differences in LVDD,LVEDV and LVESV before and after delivery in the normal pregnancy group(P<0.05).(4)The differences of GLS,GCS,gas and GRS in HDCP group,normal pregnancy group and control group before delivery were statistically significant(P<0.05),among which GLS was HDCP group<normal pregnancy group<control group,GCS,gas and GRS were HDCP group and control group,normal pregnancy group and control group(P<0.05).There was only a significant difference in GLS between the HDCP group and the control group(P<0.05).(5)There were significant differences in GRS,GLS,GCS and gas before and after delivery in HDCP group(P<0.05).There was no significant difference in GLS,GCS,gas and GRS before and after delivery in the normal pregnancy group(P>0.05).Conclusions(1)Gestational hypertension and normal pregnant women in the third trimester of pregnancy had changes in cardiac structure and function.(2)The strain values of GLS,GCS,GAS and GRS can reflect the changes in the cardiac function of pregnant women with gestational hypertension and normal pregnancy.(3)Three-dimensional spot tracking technology can objectively evaluate the myocardial deformation and systolic function,and provides a new method for the change of left ventricular systolic function of pregnant women with hypertension and normal pregnant women before and after delivery.It is expected to become a practical and effective detection tool in clinic through the innovation and exploration of STI technology in the future.
