Objective To evaluate left ventricle systolic function in patients with different grade of coronary artery lesion by speckle tracking imaging of three-dimensional echocardiography(3DE-STI),explore the clinical application value of each strain parameter. Methods 1、Subjects:40 patients of coronary heart disease without left ventricular segmental wall motion abnormality in conventional echocardiography. 27 healthy volunteers were as control group. The experimental group were divided into four groups according to the result of coronary artery angiography. 2、Equipments: Using GE Vivid 9 ultrasound system with four dimensional probe and Echo PAC work system. 3 、 Measurement: All subjects underwent conventional echocardiography and the assessment of the left ventricular strain by 3DE-STI and 2D-STI, sequentially in the same examination. Analyzed the correlation among the global area strain, Gensini score and ejection fraction. Results 1、No left ventricular segmental wall motion abnormality were found in conventional echocardiography. 2、Compared with the control group and the single-vessel lesion with collateral circulation group, LS were lower(p<0.05) in the single-vessel lesion without collateral circulation group. LS, RS, CS and AS were lower(p<0.05) in the multiple-vessel lesion group. 3、The global area strain had the biggest area under ROC curve. When the truncated value was-27.63%, the sensitivity and specificity was 70%, 91% respectively.4 、 The global area strain was negatively correlated with the Gensini score(r=-0.902,P<0.05). 5、The global area strain was positively correlated with the ejection fraction(r=0.962, P<0.05).Conclusion 1、3DE-STI can show reduced strain values of left ventricle in patients without abnormal wall motion by conventional echocardiography. It will help to determine the extent of coronary artery lesion in patients with coronary heart disease. 2、GAS has the highest accuracy to diagnose coronary heart disease. |
