Objective:To analyze the relationship between FIB,RDW and PDW in pregnancy and gestational diabetes mellitus(GDM),and to explore the clinical value of FIB,RDW and PDW levels in predicting the risk of GDM.It provides reference for clinical screening of gestational diabetes and early intervention treatment.Methods:Selecting 363 cases of pregnant women who were the delivery in the Hospital of Yanbian University from May 1,2016 to September 31,2019.According to their blood glucose status,143 healthy pregnant women were divided into the normal control group(group A).There were 220 patients with gestational diabetes,among which 116 patients were managed by nutrition control,namely controlling diet and moderate exercise to regulate blood glucose level(group B1).104 patients needed insulin to control blood glucose level(group B2).Record on general data such as age,gestational age,height,body weight before delivery,etc.record fibrinogen results in coagulation time and blood routine examination results,focusing on red blood cell count,hemoglobin content,red blood cell volume distribution width,hematocrit,platelet count,platelet volume distribution width,platelet hematocrit,etc The weight and general state of the newborns after delivery were recorded.The values of FIB,RDW,PDW and other parameters of red blood cells and platelets in predicting GDM were compared and analyzed.Results:(1)The age and BMI of group B2 were higher than those of group A and group B1,and the difference was statistically significant.Fibrinogen in group B1 and group B2 was higher than that in group A,and the difference was statistically significant.The gestational age of group B1 and group B2 was lower than that of group A,and the difference was statistically significant.The Hb and HCT levels of group B2 were lower than those of group A and group B1,and the difference was statistically significant.The levels of PDW and MPV in group B1 were higher than that in group A,and the difference was statistically significant.(2)FIB was positively correlated with BMI,PLT,PDW and PCT,but negatively correlated with MCV.RDW was positively correlated with MCV and negatively correlated with RBC.PDW was positively correlated with RBC,Hb,MPV,and negatively correlated with PLT.(3)Age,Hb,the MCV and FIB are the influence factors of gestational diabetes(P<0.05),the regression coefficients are:0.099,0.058,0.094,1.920.(4)The area under the ROC curve of FIB for the diagnosis of gestational diabetes was 0.793,the sensitivity was 50.7%,and the specificity was 94.5%,thcritical value was 5.21g/L.The areas under the ROC curve of RDW,PDW,age,gestational age,Hb,and HCT for the diagnosis of gestational diabetes were 0.474,0.572,0.584,0.419,0.552,and 0.478,respectively.Conclusion:(1)FIB can be used as an indicator to predict gestational diabetes.When FIB is 5.21g/L,the prediction effect is the best.(2)Age,Hb,MCV and FIB may be the influencing factors of gestational diabetes mellitus.(3)RDW and PDW have little reference value in predicting the occurrence of gestational diabetes mellitus.
