| Objective:To investigate whether there are differences between the two different surgical approaches in laparoscopic total gastrectomy for patients with different body mass scores during and after surgery and whether there are complications.Methods:The clinical data of patients who underwent total gastrectomy for gastric cancer in the second department of gastrointestinal surgery of the First Affiliated Hospital of Xiamen University from March 2010 to March 2019 were retrospectively collected.The two groups were compared in terms of BMI,tumor diameter,TNM stage,operation time,intraoperative blood loss,number of lymph node dissections,first exhaust time,first meal time,painkiller use time,complications,hospitalization time,hospitalization cost and postoperative survival status.Results:There was no statistically significant difference in the performance of the two groups in the normal body mass score group during the intraoperative,short-term and long-term effects.The operation time of the R-LTG group was(190 ± 18)minutes shorter(P=0.005)than the(233 ± 24)minutes of the L-LTG group,and the intraoperative blood loss was(72 ± 30)compared with the L-LTG group,the intraoperative blood loss was(85 ± 15)ml(P=0.037),and the number of lymph node dissections(47 ± 8)was compared with that of the L-LTG group(34±6)fewer(P<0.001),the time of the first meal days(3.2±0.6)was shorter(P=0.002)than that of the L-LTG group days(4.4 ± 0.7),and the differences in performance were all has statistical significance.There were no significant differences in the time of first exhaust,the days of analgesics,the time of hospital stay,the number of postoperative complications,the cost of hospitalization,and the survival status of postoperative patients.Conclusion:The right approach is safe,effective,and feasible for laparoscopic radical gastrectomy.Especially for overweight patients,the right approach is more beneficial to patients. |
