[Background and Purpose]Primary liver cancer is a high-frequency tumor worldwide.There is no effective method for early diagnosis.Most patients are diagnosed in the middle and late stages with high malignancy and difficult treatment.Although the diagnosis and treatment of liver cancer have been continuously developed in the past 20 years,liver transplantation and Surgical resection is still the first choice under the circumstances.However,it is still difficult for patients to obtain good long-term survival after surgery.The recurrence rate and mortality are still high.Accurate prognosis prediction is extremely important in patients with postoperative individualized treatment.It is important,but the clinical application of the effective biomarkers on HCC recurrence in patients is still scarce.Therefore,searching for effective biomarkers related to prognosis is of great significance to improve the survival of patients after surgery.[Methods]In this study,77 patients with liver cancer and adjacent normal tissue paraffin-embedded specimens were collected from HCC patients undergoing surgical resection at Zhongshan Hospital affiliated to Xiamen University,and all were used as self-control.Immunohistochemical staining was performed on AMACR and Ki-67,and the expression of liver cancer tissues and adjacent tissues were counted.SPSS 24.0 was used for statistical analysis of expression,clinicopathological data and follow-up data.[Result]We found that approximately 72%(n=56)of AMACR was lower expression in liver cancer,approximately 35%(n=27)was lower expression adjacent to the cancer,and Ki-67 was approximately 64%(n=50)lower expression in liver cancer.Almost no expression in adjacent tissues.The expression of AMACR in liver cancer tissues was correlated with alpha-fetoprotein(P=0.006),and also significantly correlated with the expression of KI-67(P=0.004).The low expression of AMACR in liver cancer is significantly related to DFS and OS.The risk of postoperative recurrence and death is higher than that in the high expression group(P<0.05).Patients with high Ki-67 expression in cancer have shorter DFS and OS time.The risk of postoperative relapse and death was higher than that of the low expression group(P<0.05).In addition,tumor size,number of tumors,vascular tumor thrombi,local lymph node metastasis,hepatitis B virus DNA load,and AFP level were related to postoperative recurrence(P<0.10).The expression level of Ki-67 is also an independent risk factor for patients with recurrence after surgery(P<0.05).According to the hazard ratio,it can be seen that the most significant effect on recurrence is Ki-67 expression level,followed by presence or absence of Vascular emboli,tumor size,and number of tumors.The age of patients,the presence or absence of vascular tumor thrombus,the level of AFP,the expression of AMACR and KI-67 were related to long-term survival(P<0.10).[Conclusion]The expression of AMACR and KI-67 in liver cancer is related to the recurrence and death of liver cancer after surgery.Patients with low expression of AMACR and high expression of Ki-67 have shorter tumor-free survival time and overall survival time,which can be used in combination to determine prognosis.It is conducive to early intervention and individualized treatment of patients,and has positive significance for improving long-term survival of patients.
