| Objective:Objective to investigate the mechanism of Cangxi Tongbi Capsule in the regulation of ADAMTS-4 inhibition of cartilage matrix degradation in knee osteoarthritis.Method:1.In this study,60 patients with knee osteoarthritis who met the diagnostic criteria,inclusion criteria and exclusion criteria were selected.After grouping,they were given Cangxi tongbi Capsule(approved by Shandong Pharmaceutical Administration No.Z20130043)and Jinwu Gutong Capsule orally.The degree of joint pain(VAS score method)was recorded before,after 4 and 8 weeks,respectively.The scores were compared before and after treatment,and the clinical efficacy of Cangxi Tongbi Capsule in the treatment of knee osteoarthritis was evaluated systematically and objectively.2.In this study,the knee articular chondrocytes of 1 week old SD rats were isolated by primary culture method,and induced by IL-1β as degenerative articular chondrocytes,which were divided into blank group(KB)and model group(MX).Cangxi Tongbi Capsule Group(CX),SB203580 Solution Group(SB),Cangxi Tongbi Capsule+SB203580 Group(CS),3 bottles in each group,using Western Blot and real-time PCR to detect each group of cells Differences in expression of AD AMTS-4.Result:1.VAS score:There was a statistically significant difference in VAS score between the two groups before treatment(p<0.05)compared with the fourth and eighth weeks of treatment.There was no significant difference in VAS scores between the two groups before treatment and at the 4th week of treatment(p>0.05).At the 8th week of treatment,the difference in VAS scores between the two groups was statistically significant(p<0.05),and treatment The treatment effect of the group was superior to the control group2.WOMCA score:There was a statistically significant difference in WOMAC scores between the two groups before treatment(p<0.05)compared with the fourth and eighth weeks of treatment.There was no significant difference in WOMAC scores between the two groups before treatment(p>0.05).At the 4th and 8th week of treatment,the difference of WOMAC scores between the two groups was statistically significant(p<0.05),and the treatment group was effective.Better than the control group.3.Clinical efficacy of TCM:The efficacy of the two groups was compared,the difference was statistically significant(p<0.05).The total effective rate of the treatment group(90.0%)was higher than the total effective rate of the control group(76.7%).4.The results of Western blot and real-time PCR showed that the expression level of ADAMTS-4 was the lowest in the blank group and the highest in the model group(MX);the Cangxi Tongbi capsule group,SB203580 group and Cangxi Tongbi capsule plus SB203580.The expression of ADAMTS-4 in degenerative chondrocytes was significantly reduced in the group,but the expression level of ADAMTS-4 and ADAMTS-4 mRNA were the lowest in the combination of Cangxi Tongbi Capsule and p38MAPK signaling pathway blocker(p<0.05).It is indicated that Cangxi Tongbi Capsule and SB203580 all have the effect of changing the expression level of related genes in the p38MAPK signaling pathway of chondrocytes.However,the effect of Cangxi Tongbi Capsule is weaker than that of SB203580.ConclusionOral Cangxi Tongbi Capsule has obvious curative effect on improving the symptoms of knee osteoarthritis.The mechanism may be achieved by targeted inhibition of the expression of ADAMTS-4 in cartilage tissue. |
