| Objective: Professor Xianghua Qi makes good use of pulse examination to explore the causes of diseases as well as their development and changes,and applies herbs based on syndrome differentiation to treat diseases [1].Professor Xianghua Qi uses sheng-xian decoction to treat all kinds of clinical diseases and syndromes with his own unique academic cognition and ideas of herbal adaptation.In the first part of this study,the types of diseases and syndromes and the addition and subtraction of drugs treated by professor qi xianghua with sheng-xian decoction are summarized through a retrospective study,so as to accumulate clinical research data for better inheritance of professor qi xiang-hua’s academic experience.In the second part of this study,applying flavored in sheng-xian decoction to cerebral infarction(qi deficiency and blood stasis syndrome)in the clinical diagnosis and treatment draws up clinical effective rate through the prospective study by observing patients with nerve function defect degree,daily life activities ability and clinical improvement of traditional Chinese medicine syndromes,in order to open the new thought of the clinical diagnosis and treatment of brain infarction patients with qi deficiency and blood stasis syndrome.Methods: In the first part of this study,we collects the inpatients admitted by professor Xianghua Qi from June 2015 to June 2016,who were treated with sheng-xian decoction and the outpatients who were treated from April 2017 to April 2018.A total of 152 cases are collected for statistical analysis of drugs addition and subtraction as well as the types of diseases and syndromes.In the second part of the study,all cases which are dignosised with cerebral infarction from Professor Xianghua Qi’s Clinic and the hospitalization of Affiliated Hospital of Shandong University of TCM from April 2018 to December 2018.Screen conforms to the qi deficiency of blood stasis syndrome of patients with 60 cases though diagnostic standard by western medicine,diagnostic criteria of traditional Chinese medicine and TCM syndrome classification standard,and adopting randomized controlled method is divided into the treatment group 30 cases and control group 30 cases.Among them,the control group was given conventional treatment of western medicine,and the treatment group was combined with flavored in sheng-xian decoction on the basis of conventional treatment of western medicine.Neurological impairment score,ADL evaluation of daily life activity ability and TCM syndrome score of apoplexy were given to the patients of the two groups before and after treatment respectively.Statistical methods were used to analyze the data obtained.Results:The first part of this study: 1.Through statistical analysis,there are 152 cases of medical cases that Professor Xianghua Qi used sheng-xian decoction with addition or subtraction to treat,involving a total of 14 diseases.Including 73 cases(48.02%)used for stroke-in the treatment of meridian qi deficiency blood stasis,38 cases(25.00%)used for vertigo disease from qi deficiency syndrome,14 cases(9.21%)used in impotent,5 cases(3.28%)used for consumptive disease,4 cases(2.63%)used for insomnia,3 cases(1.97%)used for headaches,3 cases(1.97%)used in the treatment of,2 cases(1.32%)used for cough,2 cases(1.32%)used for depression syndrome,2 cases(1.32%)is used to include a fever,2 cases(1.32%)used in the mouth,a flasher,a 2 cases(1.32%)for obstruction,One case(0.66%)was used for fibrillation,and one case(0.66%)was used for dementia.2 liters in tonga to reduce drug use frequency of the top 20 drugs according to the frequency highest decreasing in the order: radix astragali,radix platycodi,radix bupleuri,cohosh,rhizoma anemarrhenae,radix angelicae sinensis,radix glycyrrhizae,radix paeoniae alba,codonopsis,rhizoma ligustici wallichii,windproof,atractylodes,bran Fried hovenia dulcis,safflower,peach kernel,vinegar,puerarin,cornus,radix ophiopogonis,radix gentianae macrophyllae,and frequency are in 29 times or more.Among them,radix astragali and radix platycodon were used the most frequently,152 times in total,and 100.00% of the cases were used radix astragali and radix platycodon.3.Apoplexy--the first 12 types of drugs used frequently in the middle channels and collaterals from high to low are astragalus membranaceus,radix bupleuri,radix platycodonis,cohosh,rhizoma anaconitae,radix codonopsis,radix codonopsis,radix glycyrrhizae,radix paeoniae alba,ligusticum chuanxiong,fangfeng and atractylodes atractylodes,among which astragalus membranaceus,radix bupleuri and radix platycodonis are the most frequently used,accounting for 100.00%.Through the analysis of the frequency of drugs in apoplexy-zhongjingluo medical case,84 drugs with the frequency of more than 5% were found.A relatively valuable cluster combination of 13 types for the treatment of apoplexy has been formed.4.In all cases of vertigo,the top 9 drugs most frequently used in TCM were astragalus membranaceus,radix platycodonis,radix bupleuri,cohosh,anaesthesia,liquorice,angelica,radix paeoniae alba and ligusticum chuanxiong in descending order of frequency,and the frequency was more than 20 times.Among them,licorice and angelica sinensis were used most frequently outside the prescription of shengxi decoction(29 times each),and about 9.93% prescriptions used licorice and angelica sinensis.5.Association analysis of the compatibility of traditional Chinese medicine sheng-depression decoction in the treatment of apoplexy: For the top 20 pairs of confidence,all of them are 100%,Medicine is wind on respectively = > herba schizonepetae,dangshen = > poria cocos and atractylodes,angelica = > cornus and radix paeoniae alba,angelica wine = = > gentiana macrophylla and rhizoma ligustici wallichii,rhizoma ligustici wallichii = > gentiana macrophylla and radix paeoniae alba,angelica = > gentiana macrophylla and rhizoma ligustici wallichii,angelica = > gentiana macrophylla and radix paeoniae alba,windproof = > paniculate swallowwort root and hovenia dulcis,windproof = > paniculate swallowwort root and rhizoma ligustici wallichii,windproof = > paniculate swallowwort root and radix paeoniae alba,windproof = > paniculate swallowwort root and liquorice,rhizoma ligustici wallichii = > paniculate swallowwort root and White peony root,ligusticum chuanxiong => xu changqing and liquorice,white peony root => xu changqing and liquorice,angelica root => xu changqing and white peony,angelica root => xu changqing and liquorice,angelica root => xu changqing and dangshen.In the second part of the study,the total clinical efficacy of the treatment group was greater than that of the control group.The total effective rate of the treatment group was 86.67%,and that of the control group was 70.00%.The difference was statistically significant(P < 0.05).2.Before and after treatment,neurological impairment score,ADL evaluation of daily living ability and TCM syndrome score of apoplexy in the treatment group and the control group showed statistically significant differences(P < 0.05).3.No adverse drug reactions occurred in both groups,with good safety.Conclusion:1.Sheng-xian Decoction has litres of qi activity,the original square is mainly composed of radix astragali,radix bupleuri,rhizoma anemarrhenae,radix platycodi,cohosh,based on the rise in following the active ingredients and pharmacological effects of drugs in the study more and more,the clinical application range is gradually widely,is often used in such as respiratory system,circulatory system,digestive system disease diagnosis and treatment.It is also commonly used in clinical practice by tutors,mainly for the treatment of qi deficiency and qi depression.Through the analysis of professor qi xianghua’s medical cases of sheng-tang decoction,I gained a deeper understanding of my tutor’s academic thought and laid a foundation for his ideological inheritance.2.According to the observation of clinical trials,it can be concluded that flavored in sheng-xian decoction has a definite clinical effect on cerebral infarction of qi deficiency and blood stasis system,and has improved the score of neurological function defect,ADL evaluation of daily life activity ability and TCM syndrome score of apoplexy. |
