Objective To evaluate the efficacy of minimally invasive purulent removal in the treatment of abscess of breast carbuncle by studying the cases of abscess of breast carbuncle,and to explore the advantages of minimally invasive purulent removal in shortening the course of disease,reducing the cost of treatment,alleviating the pain of patients,maintaining the beauty of breast and lactation function compared with traditional incision and purulent removal.Methods From January 2017 to December 2018,patients who were first diagnosed as abscess of breast carbuncle in the outpatient department of mammary gland and thyroid surgery and ward of Affiliated Hospital of Shandong University of Traditional Chinese Medicine were collected.According to the order of visiting time,the patients were randomly divided into treatment group and control group.The treatment group was treated by minimally invasive purulent evacuation method,while the control group was treated by traditional incision and abscess evacuation method.The differences in treatment days,treatment costs,pain scores at the first dressing change,the rate of continued breast feeding after operation,and the satisfaction degree of patients with breast appearance.Results A total of 93 patients were collected,47 in the treatment group and 46 in the control group.There was no statistical difference between the two groups in terms of general data and the incidence of the disease before treatment.Temperature and time of blood picture returning to normal:There was no statistical difference between the two groups in the time of body temperature returning to normal(P=0.370>0.05)and the time of blood picture returning to normal(P=0.742>0.05);Cure days were 8.45±3.006 days in the treatment group and 14.11±4.581 days in the control group,P=0.000<0.05 the total treatment cost was 2376.42±975.845 yuan in the treatment group.the average in the control group was 5179.10±1535.973 yuan,P=0.000<0.05,with statistical difference;the pain score in the first dressing change:the average in the treatment group was 3.79±1.805 points,the average in the control group was 5.76±1.890 points,P=0.000<0.05,with statistical difference;the rate of continued breastfeeding after operation:85.1%in the treatment group,30.4%in the control group,P=0.000<0.05,with statistical difference;the treatment group and the control group had statistical difference;There was no breast leakage in the control group after operation.One month later,there was no recurrence in the treatment group(recurrence rate was 0%)and 1 case in the control group(recurrence rate was 2.1%),P=1.000>0.05.There was no statistical difference.Patients’ satisfaction with breast appearance:the average score of treatment group was 3.62±1.054,while the average score of control group was 1.85±0.698,P=0.000<0.05,with statistical difference.Conelus ion All the patients in both groups were cured without adverse reactions.There was no significant difference in cure rate,breast leakage rate and recurrence rate between the two groups,which indicated that both treatments had good clinical effect on breast abscess.However,the minimally invasive purulent method has more advantages in shortening the treatment time,reducing the cost of treatment,alleviating the pain of treatment,protecting breast appearance and lactation function,which indicates that the minimally invasive purulent method in this study can be popularized and applied in clinical practice.
