Abstract/Summary:
Objective:Through clinical investigation, this study collected the shandong university of Chinese medicine affiliated hospital endocrine division between January 2017 and December 2018 hospitalized patients with diabetes peripheral neuropathy of four diagnostic information and prescription information, the formulas of application spss22.0 software cluster analysis and frequency analysis, the information collected in the statistical analysis of data, generalize the common syndromes, the preliminary discussion on DPN characteristics of TCM syndrome type distribution and using the formulas, provide objective basis for differentiation treatment of DPN.Methods:Through consulting literature and combining the expert opinion, the design information collection table, diabetic peripheral neuropathy patients from shandong university of Chinese medicine affiliated hospital endocrinology in hospitalized patients collected into standard exclusion standard patient’s four diagnostic information and prescription information, the formulas of patient information using spss22.0 software on frequency analysis and cluster analysis, summarizes the distribution and justification of DPN medication regularly.Results:(1)In this study, the male to female ratio of 200 DPN patients was 1.35:1,with an average age of 57.78 and 10.357 years.(2)187 patients had abnormal vibration sensation, accounting for 93.5% of the total study population.(3)The most common symptoms of DPN patients are numbness in the limbs and dry mouth. The most common tongue color is purple and dark, the tongue shape is fat,the tongue coating is less, and the most common pulse condition is fine.(4)Through cluster analysis,it was found that there were 5 syndromes of qi and Yin deficiency(62 cases, accounting for 31.0%),damp-heat obstruction of collaterals(56 cases, accounting for 28.0%),phlegm and blood stasis obstruction of collaterals(41 cases, accounting for 20.5%),liver and kidney deficiency(31 cases, accounting for 15.5%),Yang deficiency and cold coagulation(10 cases, accounting for 5.0%).(5)There are 57 prescriptions applied in total, and the most frequently used are huanglian wendan decoction of huangqi guizhi wuwu decoction of shengmai powder.(6)The frequency of TCM taken by the patients was 3235 times, involving 241 herbs,37 herbs used more than 30 times, and the first 5 were radix astragali, radix coptidis and radix scutellariae. All the 241 traditional Chinese medicines were classified into 17 categories, and the most commonly used ones were tonic for deficiency,heat-clearing and bloodactivating drugs to remove blood stasis.Conclusion:(1)There was no significant difference in the proportion of male and female patients with diabetic peripheral neuropathy, and the prevalence of DPN was significantly increased in patients over 40 years old.(2)Most DPN patients have abnormal vibration sensation, followed by abnormal acupuncture pain sensation and temperature sensation, indicating the presence of sensory nerve damage.(3)The most common syndrome type of DPN is deficiency of qi and Yin,followed by damp-heat obstruction of collaterals, phlegm and blood stasis obstruction of collaterals, and deficiency of liver and kidney,Yang deficiency and cold coagulation.(4)In the clinical treatment of DPN,the main principle is to invigorate qi,nourish Yin,clear heat, remove dampness, dissipate phlegm and remove blood stasis, and to supplement liver and kidney with warming Yang and dispersing cold. This study summarized the clinical syndrome types and medication rules of DPN,providing a theoretical basis for the dialectical treatment of DPN.
