| Purpose and significance: the emergence of chronic osteomyelitis is a more and more clinical and clinical doctors treatment more difficult refractory diseases,multifarious,illness duration long,easy to concurrent antrum,scar,osteonecrosis,bone defect,and the operation failure and infection recurrence rate higher characteristic,become orthopaedics doctors for many years continuously explore in order to solve a problem.In this study,chronic osteomyelitis was discussed,and the clinical efficacy of traditional Chinese medicine combined with bone transfer technology in the treatment of chronic osteomyelitis was observed,so as to provide a reference for further improving the clinical treatment plan.Based on the clinical experience and clinical data of the tutor for decades,this study aims to treat chronic osteomyelitis All kinds of literature to collect the system,and combining with the affiliated hospital of shandong university of TCM orthopedic ward of follow-up survey in patients with chronic osteomyelitis,retrospective analysis and experience summary,according to the special orthopaedic biomechanics characteristics and mentor years clinical treatment of chronic osteomyelitis,analyze the treatment effect,and the use of traditional Chinese medicine(TCM)and bone transport problems do preliminary discussion and argument,makes every effort to find a feasible and standard treatment,as much as possible to shorten the treatment process of long,relieve patients’ pain,improve the level of treatment,patients with better service.Methods: this study was conducted from September 2016 to 2018 in the department of orthopedics,affiliated hospital of shandong university of traditional Chinese medicine April treated under the combination of traditional Chinese medicine internal and external bone handling technology for the treatment of chronic osteomyelitis after lesion resection of bone defects of bone from6 to 15 cm range of cases of patients were retrospectively study,explore prevention of postoperative infection,bone transport specific time late,limb function exercise method,judgment of bone healing and the intensity of mineralization,such as the best time of demolition of external fixator.Twenty-eight patients who met the inclusion criteria were randomly divided into two groups: the Chinese medicine group and the control group through a randomized controlled clinical trial.Then the infection prevention,local secretions and granulation tissue,specific time of bone transport,functional exercise method of the affected limb,determination of bone healing and mineralization intensity in later stage,and the best time for removing the external fixator were observed.Finally,the data collected are analyzed.Results: 28 cases were successfully followed up in the two groups,with an average follow-up time of one year.In the end,all the selected patients achieved satisfactory therapeutic effects in clinical practice.Among them,the soft tissue of the wound surface was effective in both groups,and the effect was more obvious in the traditional Chinese medicine group.The average shelf life was 191.69±26.71 days.Control group: 228.53±23.52 days,the difference was statistically significant(P = 0.038,< 0.05);Among the observation and comparison of postoperative complications,there were 14 cases with traction pain(5 cases in the traditional Chinese medicine group and 9cases in the control group),and the pain was gradually reduced after the removal was stopped for 2 days or the analgesic drugs such as lugek and fenby were given,without affecting the later removal treatment.Nail infection: there were 7 cases in total,including 2 cases in the TCM group and 5 cases in the control group.5 cases were controlled by local dressing change,and 2 cases were finally controlled by repeated debridement.According to statistical analysis,the number of common complications,such as nail infection and pain in transportation and traction,were less in the TCM group than in the control group,with a significant difference(P < 0.05).Conclusion: combined with the above research results,the treatment of chronic osteomyelitis by internal medicine,external application and external washing combined with modern bone medical technology,such as bone transfer,is of clinical efficacy to a large extent.Traditional Chinese medicine(gongying detoxifying lotion,xianfang shoumingyin addition or subtraction prescription,etc.)has a great promoting effect on the common complications of chronic osteomyelitis,such as endless wound fester,nail infection,pain,etc.,which can alleviate the pain of patients as much as possible and improve the medical level. |
