| Objective Application of virtual touch tissues imaging quantification technology(VTIQ)for detection of thyroid micro carcinoma(TMC)focus tissue shear wave velocity,to postoperative pathological results for distinguishing thyroid minimal risk level,combined with pathological analysis of lesions of the shear wave velocity,and to explore the value of new technology VTIQ in risk assessment of TMC.Methods Select the single stove routine ultrasound in patients with suspected TMC,diagnosed by fine needle aspiration cytology for TMC,preoperative VTIQ detection of lesions in S WV,detection of multiple sets of data to obtain lesion S WV maximum value,minimum value and calculated the average value,with the postoperative pathology results as the gold standard.According to pathology were divided into tape 1 group,namely single stove TMC with thyroid be transferred membrane infiltration,and neck region VI or neck with other regional lymph node metastasis,or with distant;tape 2 group,namely single stove TMC with thyroid is membrane infiltration,and without cervical lymph node metastasis and distant metastasis;tape 3 group,namely single stove TMC no thyroid membrane infiltration,and lymph node metastasis and distant transfer.According to the postoperative pathological grouping,the Preoperative TMC lesions of SWV values plotted ROC curve,calculation of SWV values of sensitivity and specificity,analysis between the groups of diagnostic reference critical value,to evaluate the value VTIQ technology of TMC risk assessment.Results 1.According to the results of pathology group,50 cases of TMC patients,11 cases of tape 1 group,10 cases of tape 2 group,29 cases of tape 3 group.The SWV value of tape 1 group:maximum value is(4.01±0.92)m/s,average(3.91±0.85),the minimum value is(3.75±0.76)m/s.The SWV value of tape 2 group:maximum value is(3.36±0.38)m/s,average(3.21 ±0.26)m/s,the minimum value is(3.00±0.18).The SWV value of tape 3 group:maximum value is(2.67±0.14)m/s,average(2.53±0.15)m/s,the minimum value is(2.41 ±0.17).The lesions between each risk group SWV maximum value,minimum value and average value difference,the difference was statistically significant(P<0.05).2.The SWV of maximum value,minimum value and average value in the area of ROC curve of TMC in differential diagnosis of risk are 0.952,0.994 and 0.973.The best diagnostic value between each risk group is the minimum value of SWV.3.To find out the critical value of differential diagnosis from the Receiver operating characteristic curve(ROC):The critical value of the best differential diagnosis between tape 1 group and tape 2 group is SWV minimum value 3.25m/s,and the diagnostic sensitivity,specificity and Youden index are 91%,90%,81%;The critical value of the best differential diagnosis between tape 2 group and tape 3 group is SWV minimum value 2.76m/s,and the diagnostic sensitivity,specificity and Youden index are 94%,98%,92%.Conclusions 1.VTIQ real-time shear wave elasticity imaging technique can be used to detect the SWV value of small thyroid carcinoma lesions in real time.2.VTIQ real-time shear wave elasticity imaging can provide data for the risk assessment of thyroid micro carcinoma.3.The critical value of the differential diagnosis between the tape 1 group,the tape 2 group and the tape 3 group are SWV minimum value 3.25m/s and 2.76m/s. |
