Multivariate Analysis Of Related Risk Factors Of Postoperative Complications Of Radical Surgery Rectal Cancer

Objective :To investigate the risk factors of postoperative complications of radical surgery for rectal cancerMethods: The clinical data of 562 patients with rectal cancer performe d radical resection from July 2010 to July 2014 in Affiliated Hospital of Q ingdao University were analyzed retrospectively,included patients’ basic dat a、preoperative comorbidity、operation、oncology parameter and so on,wh ich created database.Post-operative complications were graded on the basis of the Clavien-Dindo classification system.Logistic regression model was u sed to analyze the risk factors for complications of radical surgery of rect al cancer.Results1.Of 562 cases: 316 cases were male,236 cases were female.Mean a ge was 29-93(61.85±11.79).221 cases were underwent laparoscopic surgery,341 cases were underwent open surgery.2.Mean operative time was 75-495min(161.41±54.32),mean operative blood was 10-1250ml(M=200),12 cases were conversion to open surgery.3.89 cases were happened complications and the incidencerate was 15.84%.According to the Clavien-Dindo classification system for Postoperati ve complications,postoperative complications of grade I、II、III-a 、III-b、IV-a were 4.2%、10.3%、2.1%、1.9%、1.5%,respectively.No complications of IV-b were happen-ed.2 cases were dead.4.Multivariate analysis indicated that the concurrent pulmonary dise ase(OR=3.740,95%CI:1.557-8.713,P=0.005)、the time of surgery(OR=3.049,95%CI:1.244-18.015,P=0.018)and the concurrent diabetes(OR=2.502,95%CI:1.298-2.514,P=0.039)are the independent risk factors of the complications o f radical surgery for rectal cancer.Conclusions1.Our study indicated that the concurrent pulmonary disease and di abetes、the time of surgery ≥180min can increase the rate of complications of radical surgery for rectal cancer.2.Our study indicated that improve pulmonary function and monitor blood glucose of patients were important for surgery and outcome.Laparo scopic surgery requires strict pre-operative evaluation and conversion to op en surgery is needed for difficulty.The surgeon with experience in laparos copic surgery can shorten the time of surgery and increase complications.
