| Background The incidence of diabetes increased year by year and tend to be younger now,late diabetic foot is one of the serious complications of diabetes,it is hard to get effective treatment,because diabetes is a systemic disease,to comprehensively consider the effect of diabetes on multiple body systems,so as to develop a specific and useful treatment is the most effective way for the treatment of diabetic foot.Now clinically for the treatment of diabetic foot is generally debridement and dressing change,make the wound to heal,to can not heal the wound,after debridement dressing on fresh granulation wounds in thick skin piece,the transfer of full thickness skin or skin flap repair,the process is generally more slowly,is a suffering for patients’ physical and psychological process.Therefore,if clinical treatment can shorten the process,and in the process of treatment can reduce the pain of patients as much as possible,so that patients are satisfied with the treatment effect and treatment process,this is the focus of this study.In 2014,Ghanaati and some other scholars successfully prepared a new generation of platelet products by using a lower speed centrifuge(running at a speed of1500r/min for 14 minutes)and centrifuging outer whole blood without additives.Advanced Platelet-rich fibrin(A-PRF)is A platelet concentrate with more white blood cells.It is more powerful than the previous generation of platelet products and is widely used in many fields.A-PRF promotes tissue repair.This has been proven a long time ago.Artificial leather(Nike)can be used immediately after formed by traumatic wounds,can also be applied in chronic wounds,is a good temporary skin substitutes,because there is no from the other parts of the body skin,so you can largely reduce the damage,at the same time,artificial dermis mobility and plasticity is good,can be convenient to observe the growth of real leather.The purpose of this study is to observe the repair effect of A-PRF combined with artificial dermis on diabetic foot wounds through clinical application,and confirm that A-PRF combined with artificial dermis can promote and accelerate wound healing and relieve postoperative pain,thus greatly reducing the psychological pressure of patients.It adds a new technique and material application for the clinical repair of diabetic foot wounds.Methods A total of 90 patients with diabetic foot admitted to our hospital from August2019 to August 2020 were selected as the research objects.They were randomly divided into control group(45 cases)and observation group(45 cases).The control group was treated with traditional dressing change,including washing the wound repeatedly with normal saline,hydrogen peroxide and iodophor each time,covering the wound with silver ion dressing and pressurized dressing with bandages.On the basis of the above dressing changes,the observation group was treated with A-PRF,and artificial dermis was added after granulation tissue was grown.After the growth of granulation tissue was satisfied,skin grafting or skin flap transfer and repair were performed.Finally,the treatment effect,postoperative pain,times of dressing change,hospital stay,wound healing time and satisfactory growth time of granulation tissue were observed and analyzed in the two groupsResults(1)Postoperative pain assessment Visual analogue scale(VAS)was used to evaluate the wound pain of the two groups at 7d,10 d and 14 d after surgery: VAS scores of the observation group at 7d,10 d and 14 d after surgery were lower than those of the observation group(P< 0.05),and the difference was statistically significant.(2)The time of wound healing in the two groups after treatment was counted: the observation group was shorter than the control group(P< 0.05),the difference was statistically significant.(3)The frequency of dressing change after debridement was counted in the two groups:the observation group was significantly less than the control group(P<0.05),the difference was statistically significant.(4)The wound healing rate of the observation group was higher than that of the control group(P<0.05),the difference was statistically significant.Conclusion A-PRF combined with artificial dermis can accelerate wound healing,relieve pain,reduce scar and improve wound healing. |
