Abstract/Summary:
[Objective]To observe whether there is a correlation between hippocampal and/or thalamic volume and outcome after microvascular decompression for primary trigeminal neuralgia.[Methods]We retrospectively studied primary trigeminal neuralgia patients undergoing surgical treatment with microvascular decompression.Preoperative magnetic resonance imaging was used for automated segmentation of hippocampus and thalamus.Subcortical structure volumes were compared between responders and non-responders and assessed whether there is a correlation with surgical outcomes.[Results]In all,206 trigeminal neuralgia patients treated surgically from May 2016 to December 2020 were identified.A total of 136 patients met the inclusion criteria.Across all patients,thalamus volume was reduced ipsilateral compared to contralateral to the side of pain.There was no difference in hippocampal volume between the two sides.Between responders and non-responders,non-responders exhibited larger hippocampus volume(P<0.05),but thalamic volumes were not significantly different.Through receiver-operator characteristic curve analyses,found a possible correlation between hippocampal volume and surgical outcome in TN patients(AUC>0.7).[Conclusion]Preoperative hippocampal volume,measured on standard clinical magnetic resonance images,may have a correlation with surgical treatment outcome for primary trigeminal neuralgia.There may have not been a correlation between thalamic volume and surgical treatment outcome.Treatment resistance in medically refractory trigeminal neuralgia may depend on the structural features involved in limbic components of chronic pain.
