Objective:To explore the protective effects of HPG solution on ischemia-reperfusion injury of donor lung in vitro.Methods:Sixteen adult male beagle dogs were randomly divided into two groups(HPG group,LPD group),and the dogs were anesthetized then with a""U""thoracotomy for in vivo flush,then removed for cold preservation for 12-hour,after which isolated lung perfusion was performed.Then,the oxygen index,DPPC/TPL,lung compliance,white blood cell count,foam index,MPO,MDA,IL-1βmRNA,IL-6,TNF-αand immunohistochemistry image of the caspase-3 as well as HE staining results were compared between the HPG group and the LPD group with the samples were collected at 30 min,2-hour,and 4-hour after reperfusion.Result:1.The operation was successful and the lung ischemia-reperfusion in vitro model was successfully established.There was no embolism or air leakage during perfusion.2.Pulmonary function after reperfusion in both groups gradually weakened with time,and at reperfusion for 30 min after 12-hour cold preservation,the oxygenation index in both groups was greater than 300,but at 2-hour of reperfusion,the oxygenation index in the LPD group had already decreased to 263.6±26.8,while the HPG group was still greater than 300,and at 4-hour of reperfusion,the HPG group also decreased below 300(291.7±27.1),and the difference between the two groups was significant(p<0.05).In terms of foam stability index,DPPC/TPL as well as lung compliance,a time-dependent decreasing trend was observed in both groups,but the HPG group was higher than the LPD group at 30min,2-hour and 4-hour of reperfusion and the difference was significant(p<0.05).Both indicated superiority of the HPG group over the LPD group in the preservation of lung function.3.The white blood cell count,IL-6,TNF-α,IL-1mRNA,MDA and MPO of the two groups increased with the increase of time,indicating that the inflammatory reaction,tissue injury and peroxidation of the two groups were gradually aggravated,while in the comparison of the two groups,the values in the HPG group were lower than those in the LPD group at 30 min,2-hour,and 4-hour of reperfusion,and the differences were significant(p<0.05).4.At 30 min of reperfusion,HPG immunostained less caspase-3-positive expression than LPD,indicating less apoptosis than the LPD group.Under light microscopy of HE staining,a small amount of interstitial edema was observed in both groups,but alveolar interstitial red blood cell exudation was greater in the LPD group,and another part of the septal fusion into fracture and fusion into a larger cystic cavity,the destruction of which was more severe than that in the HPG group.Conclusion:With isolated lung perfusion after 12-hour of cold preservation,HPG lung preservation solution is superior to LPD preservation solution in suppressing inflammatory responses,preserving pulmonary surfactant,reduces apoptosis and preserving lung compliance. |
