| Objective Detecting the concentration of TNF-αin plasma and uterine fluid of patients with PCOS,and analyzing the expression of TNF-αin different clinical subgro ups to explore the correlation between TNF-αand PCOS.Methods A total of 98 subjects(46 PCOS patients and 52 controls)were selec ted from the Reproductive Medicine Center of the General Hospital of Ningxia Medic al University from December 2019 to December 2020.Grouping:(1)According to BMI,all subjects were divided into obese group(BMI≥25 kg/m~2)and non-obese group(B MI<25kg/m~2).(2)According to HOMA-IR,patients with PCOS were divided into insuli n resistant group(HOMA-IR≥2.69)and non-insulin resistant group(HOMA-IR<2.69).(3)According to androgen level,patients with PCOS were divided into hyperandrogenemia group(T≥45 ng/dl)and normal androgen group(T<45ng/dl).Blood was collected a nd detected for glucose,lipid profiles,insulin and sex hormones levels.And the TNF-αin plasma and uterine fluid were measured by using commercial enzyme-linked immun osorbent assay kits.Results1.General situation comparison(1)AMH,LH,LH/FSH and T in PCOS group were significantly higher than control group(P<0.01);T in PCOS patients with IR was significantly higher than PCOS patients without IR(P<0.01).(2)HOMA-IR in obese PCOS patients were higher than non-obese PCOS patients(P<0.01);FIns and HOMA-IR in PCOS patients with HA were higher than PCOS pati ents without HA(P<0.05).(3)TG and LDL in PCOS group were higher than control group(P<0.05 or P<0.01),while HDL was significantly lower(P<0.01);TG in obese PCOS patients was signi ficantly higher than non-obese PCOS patients(P<0.01),while HDL was lower(P<0.05);TG,TC and LDL in PCOS patients with IR were higher than PCOS patients without I R(P<0.05 or P<0.01),while HDL was significantly lower(P<0.01);TG and LDL in PC OS patients with HA were higher than PCOS patients without HA(P<0.05).2.Comparison of TNF-αin plasma and uterine fluid(1)TNF-αin plasma and uterine fluid in patients with PCOS were significantly higher than control group(P<0.01).(2)Plasma TNF-αin obese PCOS patients was higher than non-obese PCOS patients(P<0.05);plasma TNF-αin PCOS patients with IR was significantly higher than PCOS patients without IR(P<0.01);plasma TNF-αin PCOS patients with HA was higher than PCOS patients without HA(P<0.01).3.The expression of plasma TNF-αwas higher than that in uterine fluid in both PCOS and control groups(P<0.01 or P<0.05).4.Plasma TNF-αpositively associated with T,BMI,HOMA-IR and uterine fluid TNF-αin PCOS patients(P<0.05 or P<0.01).Conclusion1.Both whole body and uterine cavity showed chronic low-grade inflammatory reaction in PCOS patients.2.Plasma TNF-αlevel was closely related to obesity,insulin resistance and hypera ndrogenism.3.The level of TNF-αin uterine fluid was not correlated with obesity,insulin resi stance and hyperandrogenism. |
