Objective: Insulin resistance and Hyperandrogenism are characteristics of many patients with polycystic ovary syndrome (PCOS). The study assesses the therapeutic effects of Diane35 and metformin in patients with polycystic ovary syndrome. Methods: Forty women with PCOS and twenty regular cycling control subjects were included in the study. After a baseline work-up including body mass index (BMI), waist: hip ratio (WHR), Ferriman-Gallwey score, the serum LH, FSH, T, A2, PRL, E2, leptin, tumor necrosis factor-a (TNF-alpha), fasting insulin and glucose concentrations and following a 75g oral glucose tolerance test (at 60 minutes ,120minutes) , the forty patients were treated with antiandrogens Diane35 and insulin-sensitizing agents metformin. At the end of the 3-month follow-up, subjects were re-evaluated. Rusults: (1) Serum LH, androgen and leptin level of the PCOS patients were significantly higher than that of the control group (P < 0.05) There is no significant difference in serum TNF-alpha levels between the two groups (P > 0.05 ) . (2). Serum leptin (r=0.527, P=0.020), testosterone(r=0. 495,0. 382, P < 0.01)concentrations and insulin sensitivity index(r=-0. 515, -0. 494, P < 0.001) were strongly associated with BMI and WHR in PCOS group. (3) The combination of them can reduce androgen, LH, and leptin level of the PCOS patients, facilitate weight reduction, ameliorate menstrual regularity and insulin sensitivity, lead to spontaneous ovulation. Conclusions: (1) Hyperandrogen, high LH and leptin level are the characteristics of most PCOS patients. (2) Obese PCOS women have more serious dysfunctions in androgen and insulin aspects than those who are not obese have. (3) The combination of the two drugs can reduce androgen, LH, and leptin level of PCOS patients, facilitate weight reduction, menstrual regularity and lead to spontaneous ovulation.
