Abstract/Summary:
Objective:To analyze the therapeutic effect and the change of hs-CRP of pralidoxime iodide continuous pump infusion combined with hemoperfusion on patients with severe organophosphorus pesticides poisoning.Methods:The patients of organophosphorus poisoning in the emergency ICU of the second hospital of Hebei Medical University from January 2015 to October 2020 were selected.After routine gastric lavage,catharsis,symptomatic support treatment.The control group got the general treatment with atropine intravenous injection and continuous pralidoxime iodide pump infusion only.The observation group got not only the treatment same with the control group,but also the hemoperfusion.To analyze the clinical effect by comparing the dosage of pralidoxime iodide,hs-CRP,awake time,recovery time of cholinesterase,hospitalization time and improvement rate of hemoperfusion group with non-hemoperfusion group.Results: There were no obvious difference between hemoperfusion group and non-hemoperfusion group about the gender ratio,age,toxicity,dosage,visiting time and ratio of plasma treatment(P>0.05).The dosage of pralidoxime iodide in hemoperfusion group was less than non-hemoperfusion group(P<0.05).The recovery time of cholinesterase in hemoperfusion group was less than that in non-hemoperfusion group(P<0.05).There was no obvious difference in hs-CRP between hemoperfusion group and non-hemoperfusion group on admission(P>0.05);after starting of treatment,hemoperfusion group was less than non-hemoperfusion group about the level of hs-CRP(P<0.05).The awake time in hemoperfusion group was less than which in non-hemoperfusion group(P<0.05).The length of hospital stay in hemoperfusion group was less than that in non-hemoperfusion group(P<0.05).The improvement rate of hemoperfusion group was 91.3%,while that of non-hemoperfusion group was 96.3%,but no significant difference between them(P>0.05).Conclusions:1.Pralidoxime iodide continuous pump infusion combined with hemoperfusion have a better effect on severe organophosphorus pesticides poisoning.2.Hemoperfusion treatment can reduce the amount of alexipharmac,promote the recovery of cholinesterase,reduce the time of coma and hospital stay of patients with severe organophosphorus pesticides poisoning.3.Hemoperfusion can effectively remove organophosphorus poison in patients,reduce the level of hs-CRP,alleviate the inflammatory damage caused by poison to organs and tissues,and prevent the disease from developing to systemic inflammatory response syndrome or more serious situations.
