Objective: To analyze the risk factors and correlation of hydrocephalus in patients with ruptured aneurysm after clipping.Methods: 212 patients with ruptured aneurysm and emergency cranioaneurysm occlusion from January 2016 to January 2019 were selected from Shan Xi Provincial People’s Hospital.According to whether there were complications of hydrocephalus,20 patients were divided into acute hydrocephalus group(20 cases),chronic hydrocephalus group(48 cases),non hydrocephalus group(144 cases).The age,gender,history of hypertension,smoking history,intraventricular hemorrhage,intracerebral hematoma,hunt Hess score,modified Fisher grade,GCS score,aneurysm location,and erythrocyte content in cerebrospinal fluid of the three groups were collected and compared.T test,chi square test and other statistical methods were used to determine the single factor analysis of hydrocephalus after aneurysmal subarachnoid hemorrhage clipping,and then the independent risk factors of hydrocephalus after aneurysmal subarachnoid hemorrhage clipping were determined by multivariate logistic regression analysis(P <0.05).Results: A single-factor comparative analysis of the acute hydrocephalus group and the non-hydrocephalus group found that the two groups had statistical differences in hypertension,GCS score,Hunt-Hess classification,aneurysm location,and red blood cell content in cerebrospinal fluid(P<0.05).Logistic regression analysis showed that high red blood cell content in the cerebrospinal fluid was an independent risk factor for acute hydrocephalus;a single factor comparison between the chronic hydrocephalus group and the non-hydrocephalus group showed that the two groups were in hypertension,GCS score,Hunt-Hess The grading,modified Fisher grading,location of aneurysm,intraventricular hemorrhage,intracerebral hematoma,and red blood cell content in cerebrospinal fluid were statistically different(P<0.05).Logistic regression analysis showed that high Hunt-Hess grade,high modified Fisher grade,and high red blood cell content in cerebrospinal fluid are independent risk factors for chronic hydrocephalus.Conclusion:High modified Fisher grade,high red blood cell content in cerebrospinal fluid,and high Hunt-Hess grade are independent risk factors for chronic hydrocephalus;high red blood cell content in cerebrospinal fluid is an independent risk factor for acute hydrocephalus.Therefore,when the above factors are found in the management of a SAH patients,it is necessary to prevent them in time to reduce the incidence of hydrocephalus.
