| Objective:Objective:To observe the clinical effect of micro-incision internal sphincterysis in the treatment of chronic anal fissure,and to explore the scientificity and feasibility of micro-incision internal sphincterysis in the treatment of chronic anal fissure,so as to provide new methods and ideas for the improvement of clinical treatment of chronic anal fissure.Method:Sixty cases of patients in the anorectal department of Jiangxi Provincial Hospital of Traditional Chinese Medicine who were consistent with the diagnosis of chronic anal fissure were selected.IBM SPSS Statistics 21.0 statistical software was used to make a random number table.The selected 60 cases of patients with chronic anal fissure were randomly divided into treatment group(n = 30)and control group(n =30).The treatment group was treated with micro-incision internal sphincterysis,while the control group was treated with open internal sphincterotomy.Preoperative general information(gender,age,course of disease and distribution of anal fissure location)and preoperative anal pain degree of patients in the two groups were statistically analyzed to exclude the influence of related factors.Careful observation and record in detail two groups of patients with intraoperative blood loss,postoperative bowel bleeding for the first time,operation time,used for24 h urine following surgery,postoperative bowel anal pain,postoperative wound infection,wound healing time,postoperative anal appearance grade,postoperative anal incontinence,relapse,safety evaluation.IBM SPSS Statistics 21.0 statistical software conducted statistical analysis on the observed data of the two groups,and P < 0.05 indicated that the difference between the two groups was statistically significant.Results:Statistical analysis showed that: 1.There was no significant difference in general information(gender,age,course of disease and distribution of anal fissure location)between the two groups(all P >0.05);2.In terms of clinical efficacy comparison,both the treatment group and the control group could benefit,and the total effective rate was 100%;3.There were no significant differences in the degree of anal pain before defecation,24 h postoperative urination,wound infection,postoperative anal incontinence score and safety evaluation between the two groups(all P > 0.05).4.The average intraoperative blood loss in the treatment group was6.60±2.71 ml,which was significantly lower than that in the control group(8.75 ± 2.19ml),and the difference between the two groups was statistically significant(P < 0.05).5.The degree of postoperative first defecation bleeding in the treatment group was significantly lighter than that in the control group,and the difference between the two groups was statistically significant(P < 0.05);6.The mean operation time of the treatment group was 12.73±2.18 min,which was significantly shorter than that of the control group(14.92±2.34min),and the difference between the two groups was statistically significant(P < 0.05).7.Compared with the control group,the degree of anal pain in the treatment group was less on the 1st,7th and 14 th day after the operation,with statistical significance(P < 0.05).8.The duration of anal pain during defecation on the first day after surgery was shorter in the treatment group,with statistical significance(P < 0.05);9.The average wound healing of the treatment group was 14.67±1.96 d,which was significantly shorter than that of the control group(18.62±2.47d),with statistical significance(P < 0.05).10.Comparison of postoperative anal appearance score showed that the treatment group had less influence on postoperative anal appearance,and the difference was statistically significant(P < 0.05).11.According to the follow-up reports of patients in both groups 3months after surgery,there was no recurrence.Conclusion: Both micro-incision internal sphincterysis and open internal sphincterotomy can effectively treat chronic anal fissure,but in terms of shortening operative time,reducing intraoperative blood loss,reducing the degree of postoperative bleeding in the first defecation,shortening wound healing time,alleviating postoperative anal pain and protecting the appearance of the anus,Mini-incision internal sphincterysis has more obvious advantages than open internal sphincterotomy.It is a minimally invasive surgery with high cure rate,short time,less pain and quick recovery. |
