Clinical Study On Intervention Effect Of Jianpi Qushi Recipe Combined With Metformin On Spleen Deficiency And Phlegm Dampness Type Polycystic Ovary Syndrome With Insulin Resistance

Objective: To observe the clinical efficacy of Jianpi Qushi Recipe combined with metformin in the treatment of Spleen Deficiency and Phlegm Dampness Type polycystic ovary syndrome with insulin resistance,and to explore its mechanism,so as to provide further theoretical basis for clinical application.Methods: From December 2019 to December 2020,66 patients with polycystic ovary syndrome of spleen Spleen Deficiency and Phlegm Dampness Type PCOS-IR who met the inclusion criteria were selected from gynecological outpatient department of the First Affiliated Hospital of Anhui University of Chinese Medicine as the observation objects.They were randomly divided into control group and treatment group,with 33 cases in each group.The control group was given metformin orally,and the treatment group was additionally given self-designed Jianpi Qushi recipe prescription for 3courses of treatment.The changes of body mass index waist-to-hip ratio,TCM symptom scores,Self-rating Anxiety Scale and Self-Rating Depression Scale were observed before and after treatment.Before and after treatment,the sex hormones:serum follicle stimulating hormone,luteinizing hormone,testosterone,LH/FSH,glucolipid metabolic index: Fasting blood glucose,Fasting insulin,Total cholesterol HOMA insulin resistance index,and inflammation index: C-reactive protein and Interleukin 6 were measured.SPSS21.0 software was used for statistical analysis.Result:1 Comparison of BMI and WHR that both BMI and WHR were significantly decreased after treatment compared with before treatment,both P<0.05,indicating that Jianpi Qushi recipe combined with metformin and metformin alone could significantly reduce BMI and WHR of patients.After treatment,the two groups were compared,P>0.05,which showed that there was no significant difference between the two groups in reducing BMI and WHR,the effect was equivalent in the two groups.2 Comparison of comprehensive curative effect and TCM symptom score showed that the total effective rate of the treatment group was 93.55%,and that of the control group was 70.00%.The total efficacy between the two groups was P<0.05,indicating that the efficacy of Jianpi Qushi recipe combined with metformin was significantly higher than that of metformin alone.After treatment,the scores of TCM symptoms of patients in both groups were significantly reduced compared with those before treatment(P<0.05),indicating that both groups can significantly improve TCM symptoms of patients.After treatment,the scores of TCM symptoms between the two groups were compared,P<0.05,indicating that the treatment group was better than the control group in improving TCM symptoms.3 Comparison of SAS and SDS scores showed that the SAS and SDS scores of patients after treatment were significantly lower than those before treatment(P<0.05),indicating that Jianpi Qushi recipe combined with metformin and metformin alone can significantly release patients’ anxiety and depression.After treatment,SAS and SDS scores were compared between the two groups(P<0.05),which indicated that the treatment group was more effective than the control group in improving patients’ anxiety and depression.4 Comparison of sex hormone indicated that both groups could significantly reduce the serum LH,T and LH/FSH levels,all P<0.05,but had no significant effect on serum FSH,all P>0.05.The comparison of LH,T and LH/FSH between the two groups after treatment showed that there was no significant difference in improving LH/FSH between the two groups(P>0.05),but there was significant difference in improving LH and T levels(P<0.05),indicating that the treatment group was more effective in improving serum LH and T levels.5 Comparison of glucose and lipid metabolism indicated that comparison between the two groups showed that serum Fasting insulin、Fasting blood-glucose、Total cholesterol and HOMA insulin resistance index of patients after treatment were significantly decreased compared with before treatment,all P<0.05,indicating that Jianpi Qushi recipe combined with metformin and metformin alone could significantly improve the level of serum glucose and lipid metabolism of patients.Compared with the control group,FBG,TC and HOMA-IR after treatment were all P<0.05,indicating that the treatment group was significantly better than the control group in improving FBG,TC and HOMA-IR,while FINS after treatment was P>0.05,indicating that there was no significant difference in improving FINS level between the two groups.6 Comparison of inflammatory factor showed that serum C-reactive protein 、Interleukin-6 levels of patients after treatment were significantly lower than those before treatment,P<0.05,indicating that serum CRP and IL-6 levels of patients in both groups could be significantly reduced.Comparison between the two groups after treatment showed P<0.05,indicating that the two administration methods had significant differences in reducing serum CRP and IL-6 levels of patients.Conclusion: Compared with the control group,the treatment group can effectively reduce BMI and WHR,relieve clinical symptoms,anxiety and depression,regulate sex hormones,glucose and lipid metabolism,and reduce serum inflammatory factors.This type of treatment has certain clinical significance.
