| Objective:To observe the serum concentration of nociceptin/orphanin FQ(N/OFQ)in patients with diabetes during perioperative period,and to explore the relationship between N/OFQ and sympathetic nervous system and myocardial injury.Methods:Sixty-six,American Society of Anesthesiologists physical statusⅠorⅡpatients,aged 60-69 years,with elective unilateral knee replacement under subarachnoid space in the Second Hospital of Shanxi Medical University from May to August 2019 were selected and divided into diabetic group(group D,n=33)and non-diabetic group(group N,n=33).The 30 min before anesthesia(T0),1 h after operation(T1)and 24 h after operation(T2)Blood glucose,serum N/OFQ,norepinephrine(NE)were measured at 30 min before anesthesia(T0),1 h after operation(T1)and 24 h after operation(T2),serum cardiac troponinⅠ(cTnⅠ)concentration were measured at T0and T2.Results:The levels of blood glucose,serum N/OFQ and c TnⅠconcentration in group D were significantly higher than those in group N,and the concentration of NE was lower than that in group N at T0(P<0.05).The levels of blood glucose,serum N/OFQ,NE concentration in group D at T1 were higher than those at T0(P<0.05).The blood glucose in group N at T1 was higher than that at T0(P<0.05),there was no significant difference of serum N/OFQ,NE from T0 to T1(P>0.05).The serum c TnⅠconcentration at T2 in two groups was higher than that at T0,and the increase in group D was greater(P<0.05).The serum N/OFQ of group D from T0to T1 was positively correlated with the change trend of blood glucose and NE(P<0.05).The serum c TnⅠof group D from T0to T2 was positively correlated with the change trend of N/OFQ,blood glucose and NE from T0 to T1(P<0.05).Conclusion:The preoperative serum N/OFQ concentration in diabetic patients undergoing unilateral knee replacement surgery was increased,and the increase in N/OFQ concentration during the operation was related to the increase in NE concentration,and these changes were related to the damage of myocardial cell during the operation. |
